Kernaussagen

Depressionen sind häufige, schwerwiegende Erkrankungen mit hohen psychosozialen Konsequenzen. Bis zu 4 % der mindestens einmal hospitalisierten Patienten sterben durch Suizid. Nur 20 – 30 % der depressiven Störungen verlaufen monophasisch, dennoch haben Depressionen bei konsequenter Ausschöpfung der verfügbaren Behandlungsmöglichkeiten eine gute Prognose.

Das wichtigste Ziel der Depressionsbehandlung ist die Remission. Antidepressiva sind bei mittel- bis schwergradigen Episoden indiziert. Eine engmaschige Kontrolle des Therapieerfolgs mittels therapeutischem Drug-Monitoring sowie Psychometrie mittels Fremd- und Selbstbeurteilungsbögen ist ebenso obligat wie eine sorgfältige Nebenwirkungsüberprüfung. Die Wirklatenz von Antidepressiva liegt bei ca. 14 Tagen. Eine Besserung in dieser Zeitspanne ist prädiktiv für den finalen Behandlungserfolg. Ein unzureichendes Ansprechen kann viele Ursachen haben, die zunächst abgeklärt werden müssen. Danach stehen weitere pharmakologische Therapiestrategien (Dosiserhöhung, Switch, Augmentation und Kombination von Antidepressiva) zur Verfügung.

Nach Eintreten einer Remission ist das Antidepressivum für mindestens 6 Monate in der Dosis weiter einzunehmen, in der eine Remission erreicht wurde. Unter bestimmten Risikokonstellationen mit hoher Rezidivwahrscheinlichkeit kann eine Weitereinnahme über Jahre nach Abwägung von Nutzen und Risiko notwendig sein.
