Rekonstruktion der Trikuspidalklappe: Indikation, Techniken und Ergebnisse

Abstract

Tricuspid regurgitation secondary to left heart disease is the most common etiology of tricuspid valve (TV) insufficiency. Therefore, the majority of the patients, who require TV surgery, undergo concomitant mitral and/or aortic valve surgery. Uncorrected moderate and severe tricuspid regurgitation may persist or even worsen after mitral valve surgery, leading to progressive heart failure and death. For patients requiring mitral valve surgery, tricuspid valve annuloplasty should be considered even in the absence of significant regurgitation, when severe annular dilatation is present. Secondary or functional TV insufficiency is primarily treated with valve reconstruction which carries a lower perioperative risk than valve replacement. Tricuspid valve repair with ring annuloplasty is associated with improved survival and a lower reoperation rate than with suture annuloplasty. Patients, who require tricuspid valve surgery either as an isolated or a combined procedure, still constitute a high-risk group. The long-term survival is poor.

  • Bei der Mehrzahl der Patienten mit Trikuspidalklappeninsuffizienz handelt es sich um eine sekundäre oder funktionelle Klappeninsuffizienz bei führender Linksherzerkrankung (z. B. Mitralklappeninsuffizienz/‑stenose). Ohne chirurgische Behandlung haben Patienten mit mittel- bis hochgradiger Insuffizienz eine 3-Jahre-Überlebenswahrscheinlichkeit von nur 40 %.

  • Eine isolierte Trikuspidalklappenrekonstruktion ist indiziert bei symptomatischen Patienten mit schwerer Insuffizienz, bevor sich eine rechtsventrikuläre Dysfunktion manifestiert. Eine klare Indikation besteht auch bei Patienten mit Trikuspidalklappeninsuffizienz und zusätzlicher Indikation zu einer Linksherzoperation (Mitral-, Aortenklappe, ACVB). Die Trikuspidalrekonstruktion wird auch empfohlen, wenn eine Ringdilatation (≥ 40 mm oder > 21 mm/m2) ohne höhergradige Insuffizienz besteht.

  • Trikuspidalklappeneingriffe finden zu 80 – 90 % in Kombination mit anderen kardialen Eingriffen (vorwiegend Mitralklappenchirurgie) statt. Der Zeitpunkt der Operation wird bestimmt durch die führende Pathologie; sie sollte jedoch durchgeführt werden, bevor die rechtsventrikuläre Pumpfunktion eingeschränkt ist und Nieren- bzw. Leberfunktionsstörungen auftreten.

  • Die Klappenrekonstruktion ist die Therapie der Wahl. Ein Klappenersatz ist selten erforderlich und Fällen vorbehalten mit primärer struktureller Klappenerkrankung und schwer destruierter Trikuspidalklappe.

  • Patienten, bei denen zur Rekonstruktion ein Anuloplastiering verwendet wird, haben verglichen mit der Nahtanuloplastie eine geringer Reoperationsinzidenz und ein besseres Langzeitüberleben.

  • Patienten, bei denen ein Trikuspidalklappeneingriff erforderlich ist, stellen ein Hochrisikokollektiv dar.

  • Eine residuelle oder neu aufgetretene Klappeninsuffizienz nach Trikuspidalklappenrekonstruktion ist bei 15 – 30 % der Patienten nachweisbar.

  • Trikuspidalklappen-Reoperationen sind mit einer hohen Letalität assoziiert.
