Zusammenfassung

Die Therapie des differenzierten Schilddrüsenkarzinoms mit I-131 kann als paradigmatisch für die Applikation einer theranostischen Substanz (Diagnostik und Therapie unter Anwendung des gleichen Vehikels) angesehen werden. Die erfolgreiche Behandlung von neuroendokrinen Tumoren nach dem THERANOSTIK-Prinzip hat den Weg auch für andere Tumorentitäten geebnet. So stellt die Kombination von molekularer Bildgebung von primären und metastasierten Prostatakarzinomen mit niedermolekulären Ga-68 markierten PSMA (prostate-specific membrane antigen) Inhibitoren mit der molekularen Radiotherapie mit Lu-177-markierten Derivaten- (Radio-Liganden-Therapie, PRLT) den nächsten Meilenstein der personalisierten Nuklearmedizin dar. Ga-68-PSMA-PET/CT ermöglicht die hochsensitive Detektion von (entdifferenzierten) Prostatakarzinomen und sehr kleinen Metastasen im Bereich von wenigen Millimetern mit hoher diagnostischer Sensitivität und Spezifität. Die quantitativen, reproduzierbaren PET/CT-Bilddatensätze können für die Selektion von Patienten für die PRLT und die Evaluation des Ansprechens auf die Therapie herangezogen werden. Wie die ersten Ergebnisse der von uns seit Anfang 2013 mit Lu-177 PSMA behandelten Patienten zeigen, ist die PRLT eine äußerst vielversprechende, anscheinend hocheffektive und dabei nahezu nebenwirkungsfreie Methode zur Therapie von fortgeschrittenen Prostatakarzinomen, die auf eine Hormon- und/oder Chemotherapie nicht mehr ansprechen, also von Patienten, die sich im Endstadium der Erkrankung befinden. Weitere, möglichst prospektive Studien müssen zukünftig den Stellenwert der peptidvermittelten Radio-Liganden-Therapie mit Lu-177 PSMA genauer definieren, insbesondere im Vergleich (oder auch in Kombination) mit bereits etablierten Therapeutika wie Enzalutamid, Arbirateron oder Docetaxel. Weitere Verbesserungen sind denkbar durch die Kombination mit antihormonellen Substanzen und die Anwendung von Radiosensitizern sowie potentiell durch eine Hyperfraktionierung.

Abstract

Radioiodine was a frontrunner as a theranostic agent for the management of thyroid cancer. The success of using theranostic pairs in neuroendocrine neoplasms has indeed paved the way to the development of targeted diagnosis and therapy in other tumors. Molecular imaging of prostate cancer using PET/CT with Ga-68 labeled prostate specific membrane antigen (PSMA) small molecules and molecular radiotherapy applying peptide radioligand therapy (PRLT) with Lu-177 labeled PSMA inhibitors is the next major milestone in personalized nuclear medicine. Ga-68 PSMA PET/CT enables very accurate detection of prostate cancer and its metastases with high diagnostic sensitivity and specificity and provides quantitative, reproducible data that can be used for selecting patients for PRLT and evaluation of therapy response. PRLT using Lu-177 PSMA inhibitors seems to be very effective for the treatment of metastasized, castrate-resistant progressive prostate cancer as shown by first results obtained in over 50 patients treated at our center since early 2013.
