Aptis-Prothese bei schmerzhafter Instabilität des Ulnastumpfes nach hoher Resektion der Ulna

Zusammenfassung

Wir berichten über eine 35-jährige Patientin mit einer ausgeprägten Madelung-Deformität, bei der es in Folge einer Kapandji-Sauvé-Operation mit hoher Resektion der Ulna zu einer schmerzhaften Instabilität des Ulnastumpfes kam. Obwohl schon eine Radiuskorrekturosteotomie durchgeführt wurde, bestand zusätzlich noch eine deutlich erhöhte Ulnarinklination des distalen Radius mit anhaltenden Schmerzen. Es erfolgte die Implantation einer gekoppelten Prothese für den Komplettersatz des distalen Radioulnargelenkes in Verbindung mit einer erneuten, extraartikulären Korrekturosteotomie des distalen Radius, wobei der Zapfen der radialen Prothesenträgerplatte im Loch der entfernten Großfragmentschraube nach Kapandji-Sauvé-Operation platziert wurde.

Abstract

We report the case of 35-year-old woman with severe Madelung’s deformity. Having undergone a Kapandji-Sauvé-procedure with wide excision of the distal ulna, the patient suffered from painful instability of the ulnar stump. Although a corrective osteotomy of the distal radius had been performed, severe ulnar inclination of the distal radius remained, leaving persisting pain. Implantation of a constrained distal radioulnar joint prosthesis combined with a renewed extraarticular corrective osteotomy of the distal radius was performed. The peg of the radial plate of the prosthesis was placed in the screw hole of the large-fragment lag screw removed after the Kapandji-Sauvé-procedure.
