Einzeitige Komplex- und Kombinationsoperationen bei Patienten des rheumatischen Formenkreises

Zusammenfassung

Die Zahl der operativen Eingriffe bei RA-Patienten ist hoch und steigt mit zunehmender Krankheitsdauer weiter an (21). Komplexoperationen (mehrere Eingriffe in einer anatomischen Region) und chirurgische Kombinationseingriffe (einzeitige beidseitige Interventionen) helfen den Patien-ten, weil sie operationsbedingte Ausfälle im Beruf und Privatleben minimieren. Die Lebensqualität steigt und psychologische Vorteile sind offenkundig. Operateur und Anästhesist führen eine individuell auf den Patienten zugeschnittene Operationsplanung durch, deren Ziel es ist, die Belastung des Patienten zu reduzieren und das operative Langzeitergebnis zu optimieren. Die anspruchsvolle postoperative Mobilisation nach einem solch komplexen Eingriff fällt in den Bereich der Physio-Ergotherapie und Or-thopädietechnik, gegebenenfalls ist die Versorgung mit speziellen Hilfsmitteln und Orthesen angezeigt. Die Zusammenarbeit mit speziell ausgebildeten und ausgerüsteten Rehabilitationseinrichtungen ist ein Eckpfeiler der postoperativen Therapie. Die berufliche Rehabilitation ist sicherzustellen. Die technisch hoch spezialisierte Versorgung der Patienten des rheumatischen Formenkreises mit Komplexund Kombinationsoperationen erfordert aufgrund der charakteristischen, entzündlich bedingten Deformitäten und des perioperativen Managements praktisch immer die Kompetenz eines rheumaorthopädischen Zentrums.

Summary

The number of surgical intervention in rheumatoid arthritis patients is high, and it increases by the duration of the ailment. Complex surgical operations (different surgical interventions in one anatomical region) and combined surgical interventions (simultaneous bilateral operations) help to minimize the morbidity both at work and in private life. The quality of life increases and the psychological advantage is obvious. The surgeon and the anaesthesiologist should plan the surgery individually for the particular patient in order to reduce the stress of surgery and to optimize the long-term results. The difficult postoperative mobilization after such a complex intervention must be supervised by the physiotherapist, ergotherapist and the technical orthopaedic assistant. Sometimes special extern devices, aids and ortheses are needed. The teamwork with specialized rehabilitation centres is a very important issue in the postoperative management. The return to professional activity is to be taken care of. Therefore, one can conclude that, because of the need for highly and technically specialized interventions, the perioperative management of rheumato-logic patients should be realised in specialised rheumato-logical orthopedic centres.
