Zusammenfassung

Motorische Defizite wie Gangstörungen und Schwierigkeiten beim Greifen treten bei vielen Multiple Sklerose (MS)Patienten auf und beeinträchtigen Alltagstätigkeiten oft erheblich. In der Therapie dieser Einschränkungen kommt der Physiotherapie eine zentrale Rolle zu. Die angewandten Verfahren waren lange nur empirisch belegt, auch wurde den Patienten über viele Jahre geraten, physische Aktivität zu meiden. Inzwischen gilt jedoch der förderliche Einfluss von körperlichem Training auf die Verbesserung der Mobilität als unumstritten, die Fatigue reduziert sich sogar. Mittels standardisierter Tests werden die Defizite erfasst und ein individueller Behandlungsplan erstellt. In der Literatur finden Physiotherapeuten Richtlinien, wie Gangstörungen durch Laufband-, Kraftund Gleichgewichtstraining nachweislich verbessert werden können. Evidenzbasierte Therapieoptionen für die Verbesserung des Greifens existieren bisher nur hinsichtlich der Reduktion von Kraftdefiziten und des Intentionstremors.

Summary

Motor deficits as difficulties in walking or grasping are very common in patients suffering from multiple sclerosis and impact activities of daily living often remarkably. Physiotherapy plays an important role in treating this impairments. For a long time the applied methods have been based on empirical data only and patients have been advised to avoid physical activity. In the meantime the positive effects of exercise training on mobility skills are indisputable, the fatigue even improves through exercises. Rating scales should be used to evaluate the deficits and to create a treatment plan. In the literature physiotherapists find evidence based guidelines how difficulties in walking can be minimised by treadmill-, resistanceand balance training. Evidence based options for enhancing manipulative functions exist only in terms of reducing muscular weakness and decreasing intentionstremor.
