    Aims: Background:

    Small bowel capsule endoscopy (SBCE) is the gold standard in the investigation of iron deficiency anemia (IDA) after negative esophagogastroduodenoscopy (EGD) and colonoscopy. IDA occurs in 2 – 5% of adult men and postmenopausal women (PsM) and is a common cause of referral to gastroenterologists. IDA in premenopausal women (PrM) is often presumed to be due gynecological causes, and investigation is generally deferred in favor of iron replacement therapy.

    Aim:

    To determine the frequency of intestinal lesions in the SBCE performed in PrM and PsM with IDA and its potential predictors.

    Methods:

    Retrospective study. Medical and SBCE records were reviewed in consecutive PrM and PsM with documented IDA who were referred for SBCE between July 2013 and October 2017, after negative EGD and colonoscopy. Gynecological disease was excluded.

    Results:

    144 patients were included, 94 (65.3%) were PsM and 50 (34.7%) PrM. The mean age was 60.03 ± 19.34 years. The overall diagnostic yield of SBCE for significant findings was 46.5%. In the PrM patients, 15 (30%) had a diagnostic SBCE and the most frequent lesions were erosions (28%) and ulcers (20%). In PsM, 52 (55.3%) had a diagnostic SBCE and the most frequent lesions were angioectasias (38.3%). The diagnostic yield of SBCE in PrM was significantly lower when compared to PsM (30% vs. 55.3%; p = 0.004).

    Moreover PsM patients with positive SBCE were older (74.02 vs. 68.81; p = 0.028) and with lower hemoglobin (7.93 vs. 9.1 g/dl; p = 0.004).

    In PrM we did not find any potential predictor factor for positive SBCE.

    Conclusions:

    The diagnostic yield of SBCE in the etiological study of IDA in PrM is lower when compared to PsM. Nevertheless, significant findings were observed in almost a third of such patients. Although we did not find any predictive factor of diagnostic SBCE in PrM, these results support the need of a proactive investigation of IDA in this group.
