Thrombozytosen im Kindesalter

Zusammenfassung

Thrombozytosen im Kindesalter galten bisher als selten und werden in der pädiatrischen Literatur nur sehr knapp und uneinheitlich dargestellt. Wenn die Thrombozytose als eine Plättchenzahlerhöhung über 500000/µl definiert ist, kann die Häufigkeit bei Routineuntersuchungen an Kinderkliniken auf 3-13% geschätzt werden. Es handelt sich fast ausschließlich um sekundäre Thrombozytosen, die als Reaktion auf Infektionen (weitaus am häufigsten), Traumen und Operationen, Hypoxämie, Immunprozesse, Frühgeburt, gastrointestinale Erkrankungen, Medikamente, Streßsituationen oder auf einen vorangegangenen Plättchenverlust entstehen. Die sekundäre Thrombozytose liegt zumeist unterhalb von 700000 Plättchen/µl und ist temporär.

Wenn nur eine Thrombozytose besteht, kommen Thrombosen praktisch nicht vor. Prophylaktische Maßnahmen sind nur dann zu erwägen, wenn andere Thromboserisikofaktoren, wie Gefäßschaden, permanenter Eisenmangel, Hyperviskosität, Immobilisation dazukommen. Primäre Thrombozytosen infolge eines Stammzelldefektes kommen im Kindesalter extrem selten vor. Bei asymptomatischen Patienten gibt es über den Wert prophylaktischer Maßnahmen keine gesicherten Erkenntnisse.
