Zusammenfassung

Anamnese | Eine 85-jährige Patientin mit einer perkutanen endoskopischen Gastrostomie (PEG) -Sonde stellt sich wegen rezidivierender abdominaler Schmerzen und wechselnden Stuhlverhaltens in der zentralen Notaufnahme vor. Die PEG-Sonde wurde im Rahmen der Behandlung eines früheren Oropharynxkarzinoms gelegt, in der Vergangenheit mehrfach gewechselt und weiterhin regelmäßig verwendet.

Untersuchungen | Die klinischen Untersuchungsbefunde inklusive Laborchemie waren bei Vorstellungen unauffällig; eine Abdomensonografie zeigte leicht distendierte Dünndarmschlingen, die Röntgenabdomenübersichtsaufnahme wurde als unauffällig interpretiert. Die Symptomatik wurde letztendlich als Subileussymptomatik bei Obstipationsneigung bewertet; die endoskopische Abklärung mittels Ösophagogastroduodenoskopie zeigte in diesem Zusammenhang eine korrekte PEG-Sondenlage bei sonst unauffälliger Schleimhautmorphologie und in einer Koloskopie stellte sich das Kolon insgesamt unauffällig dar. Eine ergänzende Computertomografie des Abdomens zeigte letztendlich im terminalen Ileum einen ca. 2,5 cm großen, flächigen Fremdkörper, welcher bei genauerer Differenzierung und retrospektiver, erneuter Begutachtung der Abdomenübersichtsaufnahme einer PEG-Halteplatte entsprach.

Therapie und Verlauf | Eine endoskopische Bergung des Fremdkörpers gelang nicht. In der nachfolgend durchgeführten Operation ließ sich eine gedeckte Perforation eines durch die PEG-Halteplatte subtotal verschlossenen Ileumsegmentes ca. 10–15 cm vor der Ileocoecalklappe nachweisen. Es erfolgte eine Ileumresektion von 10 cm und die Anlage einer Seit-zu-End-Ileo-Zökostomie. Der weitere postoperative Verlauf war unauffällig. Die Patienten konnte beschwerdefrei im Verlauf entlassen werden.

Folgerungen | In der Anamnese gab es keine Hinweise auf einen früheren Abriss der PEG-Sonde; vermutlich wurde die Halteplatte bei einem PEG-Sondenwechsel in der Vergangenheit dem Spontanabgang überlassen. Es besteht der Anspruch an den endoskopisch tätigen Gastroenterologen, dass die Anlage, der Wechsel oder die Entfernung einer PEG nur unter endoskopischer Führung erfolgen sollte.

Abstract

History and clinical findings | An 85-years-old woman with percutaneous endoscopic gastrostomy (PEG) presented at our emergency unit with repeating abdominal pain and changes in the consistency of the stool. The PEG was placed during a treatment of an oropharynxcarcinoma in the past, was replaced repeatedly and is still in use.

Investigations | The initial clinical results were unobtrusive; the ultrasound of the abdomen showed mild distended loops of the small intestine, the x-ray of the abdomen was unsuspicious with diagnosis of subileus by constipation. The esophagogastroduodenoscopy was also without pathological findings and showed correct position of the PEG and normal aspect of the gastric tissue. An additional computerized tomography (ct-scan) of the abdomen demonstrated a foreign body nearly 2.5 cm in diameter in the distal part of the small intestine, the terminal ileum, differentiated as PEG retaining plate, also seen retrospectively in the initial x-ray.

Treatment and course | Because the endoscopic salvage failed, the foreign body had to be removed surgically. The retaining plate was the cause for subtotal occlusion and also for the perforation of the small intestine. It was found nearly 15 cm before ileo-terminal valve. After operation the patient recovered soon.

Conclusion | The history gave no hints of accidentally broken PEG-tube; presumably the retaining plate was cut off while the PEG was changed and let go on natural way. The aim of gastroenterologists should be removing PEG under endoscopic control.
