Intraaortale Ballongegenpulsation in der Herzchirurgie – Aktuelle Datenlage und Leitlinienempfehlungen

Die intraaortale Ballongegenpulsation mit einer in die Aorta descendens eingebrachten intraaortalen Ballonpumpe (IABP, [Abb. 1]) wurde vor > 50 Jahren in die klinische Praxis eingeführt. Seitdem hat sie sich zum am weitesten verbreiteten und komplikationsärmsten mechanischen Kreislaufunterstützungssystem entwickelt. In jüngster Zeit wurde allerdings zunehmend Kritik laut, dass die positiven hämodynamischen Effekte der IABP und der positive Einfluss auf das Outcome deutlich überschätzt würden [1]. Dies betrifft weniger die Herzchirurgie, sondern v. a. die Kardiologie: Hier wurde die IABP viele Jahre sowohl als Unterstützungssystem während Hochrisikointerventionen [2] als auch v. a. bei Patienten im kardiogenen Schock nach Myokardinfarkt [1] eingesetzt.
