Zusammenfassung

Etwa die Hälfte aller operationspflichtigen Patienten mit Mitralklappeninsuffizienz wird aufgrund eines erhöhten perioperativen Risikos nicht operiert. Dies hat über die letzten Jahre zur Entwicklung einer Vielzahl weniger invasiver, interventioneller Behandlungsmethoden für die Mitralklappeninsuffizienz geführt. Der vorliegende Artikel fasst zum einen Diagnose und Indikationsstellung zur operativen und interventionellen Therapie der Mitralklappeninsuffizienz zusammen und gibt zum anderen einen Überblick über die neuesten Verfahren zur interventionellen Rekonstruktion, aber auch zum Transkatheter-Ersatz der Mitralklappe.

Abstract

A substantial proportion of patients suffering from mitral regurgitation are denied surgery despite an indication due to an increased perioperative risk. This had led to the development of a large number of less-invasive, interventional procedures for the treatment of mitral regurgitation. The present article summarizes the diagnostic approach as well as the indications for interventional and surgical therapies for mitral regurgitation and gives an overview of novel transcatheter strategies for repair and replacement of the mitral valve.
