Sergio Cadoni et al. Water exchange for screening colonoscopy increases adenoma detection rate: a multicenter, double-blinded, randomized controlled trial

This randomized controlled trial compared three different colon intubation techniques with respect to adenoma detection in 1224 patients. Compared with standard air insufflation, water exchange yielded a significantly higher overall adenoma detection rate (ADR; 49.3 % vs. 40.4 %), right colon ADR (24.0 % vs. 16.9 %) and advanced ADR (6.1 % vs. 2.5 %), and mean number of adenomas per procedure (0.88 vs. 0.72). Although the ADR using water immersion was numerically greater than that with air insufflation and lower than that with water exchange, these differences were not significant.
