Parodontitis vs. Periimplantitis

Nach dem Durcharbeiten dieses Artikels sollte der Leser Folgendes reflektiert haben:
▪ Jährlich werden in Deutschland etwa 500 000 Implantate inseriert, in den USA sind es ca. 2,3 Mio.
▪ Mit dem Anstieg der Implantationen steigt auch die Zahl periimplantärer Erkrankungen.
▪ Im Gegensatz zur Parodontitisbehandlung gibt es für diese bisher noch kein evidenzbasiertes Therapiekonzept.
▪ Erfahrungen zeigen, dass konventionelle parodontale Behandlungsverfahren nicht 1 : 1 auf periimplantäre Erkrankungen übertragbar sind.
▪ Der Beitrag zeigt Unterschiede und Gemeinsamkeiten der beiden Infektionserkrankungen auf und welche Bedeutung sie für die Praxis haben.

Die Periimplantitis extendiert eher Richtung apikal, hat eine höhere Dichte an vaskulären Strukturen, und inflammatorische Zellinfiltrate sind am Implantat größer als am natürlichen Zahn.

Die Mukositis führt schneller zur Periimplantitis als analog dazu die Gingivitis zur Parodontitis.

Das Entzündungsgeschehen bei der Periimplantitis ist durch Transkriptionsmechanismen reguliert, bei der Parodontitis eher durch bakterielle Antwortsysteme.

Fibroblasten von Periimplantitisläsionen zeigen im Vergleich zu Parodontitisläsionen eine erhöhte Produktion von Vaskularisationsfaktoren und eine verminderte Inhibition von Wachstumsfaktoren, die die Kollagensynthese stimulieren.

Bakterien des roten Komplexes lassen sich bei Parodontitis und Periimplantitis nachweisen, finden sich aber vermehrt bei Parodontitispatienten.

Die Periimplantitis hat eine deutlich geringere mikrobiologische Diversität als die Parodontitis, wobei z. B. Staphylococcus aureus nur bei Periimplantitis, nicht aber bei Parodontitis detektiert wurde.

Die Spiegel proinflammatorischer Interleukine sind bei der Entzündung sowohl bei Parodontitis als auch bei Periimplantitis erhöht, im direkten Vergleich am höchsten bei Periimplantitis.

Neue Ansätze bei Periimplantitis: Knochenverlust hat bei Implantaten möglicherweise nur bedingt einen bakteriellen Ursprung und könnte durch eine Fremdkörperreaktion geprägt sein.
