Technical tips for endoscopic ultrasound for pancreatic lesions in patients undergoing total gastrectomy and Roux-en-Y reconstruction
Abstract Text Endoscopic ultrasound (EUS) for pancreatic lesions in patients with operated stomachs is technically challenging. This study aimed to provide technical tips for EUS for pancreatic lesions in patients who underwent total gastrectomy and Roux-en-Y reconstruction. In this video, EUS and X-ray images are shown during pancreatic observation in such patients, and EUS-guided fine needle aspiration (EUS-FNA) is also performed. A convex echoendoscope (GF-UCT-260, Olympus, Japan) and EUS video processors (EU-ME2 and ME3, Olympus) were used. In patients who underwent total gastrectomy and Roux-en-Y reconstruction, EUS remains challenging, particularly for pancreatic head lesions. However, this may become feasible when EUS is initiated at a slightly deeper site in the efferent loop. Once a pancreatic lesion has been identified, EUS-FNA can be performed using conventional methods.
