Vergrößerungspräparateradiographie als Qualitätskontrolle der Mikrokalkerfassung im Rahmen der histopathologischen Aufarbeitung von Mammabiopsiegewebe

Zusammenfassung.

Zielsetzung: Bei der histopathologischen Aufarbeitung malignomverdächtiger mikrokalkhaltiger Mammaexzisionspräparate sind Präparateradiographien als Qualitätskontrolle hinsichtlich der vollständigen Erfassung der Mikrokalkareale durch den Pathologen kein allgemein anerkannter Standard. Ziel der Untersuchung war die Evaluierung der Präparateradiographie in Vergrößerungstechnik als Qualitätskontrolle und Navigationshilfe für den Pathologen bei der histologischen Erfassung der Mikrokalzifikationen. Methode: Die Gewebereste von 29 Mammaexzisionspräparaten wurden jeweils nach der histopathologischen Aufarbeitung mit dem Vergrößerungsmammographiesystem DIMA Plus M11 4fach radiographiert und auf das Vorhandensein von Mikrokalk überprüft. Im positiven Fall wurden die mikrokalkhaltigen Gewebeanteile erneut einer histologischen Untersuchung zugeführt. Ergebnisse: Bei 55 % (16 von 29) aller Präparate konnten wir nach der histopathologischen Aufarbeitung mit Hilfe der DIMA-Vergrößerungspräparateradiographie verbliebene Mikrokalkpartikel identifizieren. In 5 dieser 16 Bioptate war der Mikrokalk zu 100 % histopathologisch nicht erfaßt worden. In 6 Fällen wurden die intraoperativen Präparateradiographieaufnahmen an den Pathologen mitgegeben. Hierbei ließ sich die Quote mit vollständiger Erfassung der verdächtigen Areale auf 66 % (4/6) gegenüber 39 % (9/23) ohne Mitgabe der Bilder steigern. Durch histopathologische Nachuntersuchungen der Gewebeanteile mit radiologisch nachgewiesenem verbliebenen Mikrokalk konnten in einem Fall invasive Anteile eines bis dahin als DCIS (ductales Carcinoma in situ) befundeten Mammakarzinoms diagnostiziert werden. Schlußfolgerung: Unsere Untersuchungen zeigen, daß eine Präparateradiographie nicht nur intraoperativ, sondern auch im Rahmen der histologischen Aufarbeitung notwendig ist.

Purpose: Evaluation of specimen radiography as a quality control for the detection of microcalcifications by the pathologist during histopathological examination of breast biopsies. Materials and Methods: 29 breast biopsy specimen with microcalcifications were radiographed using the magnification (×4) mammography System DIMA Plus Ml l before and after the histopathological examination. Tissue with detected microcalcifications after the histopathological examination was reexamined histologically. Results: In 55 % (16/29) of all specimen we could identify missed microcalcifications. In 5 specimens the microcalcifications were missed completely. The radiologically detected areas of residual microcalcifications were reexamined histologically and in one case invasive parts of a carcinoma were detected, which primarily had been diagnosed as an in-situ carcinoma. In 6 cases the radiographs of the intraoperative specimen radiography were sent to the pathologist together with the biopsy. In these cases the rate of complete detection of the microcalcifications increased from 39 % to 66 %. Conclusion: Specimen radiography should be performed not only intraoperatively but also as part of the histopathological examination.
