Vorbemerkung
Im Gegensatz zu organischen, meist gut definierten gastroenterologischen Krankheitsbildern, wie z. B. dem Ulkusleiden und Morbus Crohn, mit klar umschriebenen diagnostischen und therapeutischen Handlungsanweisungen sind die funktionellen gastroenterologischen Störungen und Erkrankungen jeweils als Ergebnis von krankhaft veränderten Funktionen ohne Nachweis von biochemischen und/oder morphologischen Veränderungen, besonders für die Diagnosesicherung und Behandlungsstrategie schwer fassbar.
Dies trifft im besonderen Maße für das Reizdarmsyndrom (RDS) zu. In den letzten Jahren wurden von verschiedenen Fachgesellschaften mehrere Leitlinien zum Management des RDS publiziert wie der »Technical Review on IBS« aus den USA [9], die Revision des Rom-I-Projektes »The functional gastrointestinal disorders Rome II« [7] sowie der Konsensusbericht der Deutschen Gesellschaft für Verdauungs- und Stoffwechselkrankheiten (DGVS) 1999 [18] und »Guidelines of the Management of IBS der britischen Society of Gastroenterology« [23] .
Die vorliegende Übersichtsarbeit basiert im Wesentlichen auf den vor 2 Jahren erarbeiteten deutschen Leitlinien der DGVS unter Berücksichtigung neuerer Publikationen innerhalb der letzten 2 Jahre.
