Noncompliance: ein Überblick

Noncompliance untergräbt die Effizienz der Medizin durch mangelnde Ausführung der Therapie und ist daher gewissermaßen das Urproblem der Medizin überhaupt. Schon Hippokrates gab seinen Schülern den Rat, auf mögliche »Fehler und Lügen« der Patienten bei Befolgung der verordneten Maßnahmen zu achten [26]. Seit etwa 25 Jahren hat sich die Wissenschaft systematisch dieses Problems angenommen und nennt es nunmehr »Noncompliance«. Die Erfahrung der alten Ärzte wird durch die neueren Statistiken bestätigt: Nur ungefähr ein Drittel der Patienten nimmt seine Medikamente entsprechend der Verschreibung ihrer behandelnden Ärzte ein, ein weiteres Drittel nimmt seine Medikation gar nicht, und das letzte Drittel schwankt zwischen ersterem und zweitem [41]. Somit dürfte die Häufigkeit der Noncompliance - je nach Definition - um die 50 % liegen [11]. Nach Rashid [35] holen fast 20 % der Patienten die von ihrem Arzt verschriebenen Medikamente gar nicht in der Apotheke ab.
