Kreuzbandersatz in der TransFix®-Technik

Zusammenfassung

Aufgrund der mangelnden gelenknahen Fixation und der damit verbundenen Instabilität wird die Kreuzbandersatzoperation in Endobutton-Technik in den letzten Jahren zunehmend in Frage gestellt. Auch die Ersatzoperation mit dem mittleren Patellarsehnendrittel wird aufgrund der hohen Transplantatentnahmemorbidität, sowie Probleme bei der gelenknahen Fixation zunehmend kritisch diskutiert. Hier stellt die Kreuzbandersatzoperation unter Verwendung der Hamstring-Sehnen und Fixation über den TransFix®-Pin mit einer ausreißfesten femoralen Fixation sowie einer ebenfalls gelenknahen tibialen Fixation über eine biodegradible Interferenzschraube eine gute Alternative dar. Die Operationsmethode ist technisch ausgereift. In der beschriebenen Technik der Kreuzbandersatzoperation (TransFix®- Technik) wurden in der Klinik für Unfallchirurgie des Klinikums Nürnberg vom 01. 10. 1998 bis zum 30. 06. 2000 insgesamt 148 Patienten operiert. Von diesen 148 Patienten konnten bislang 32 im Rahmen der Ein-Jahres-Kontrolle nachuntersucht werden. Die Bewertung im IKDC-Score ordnet 8 Patienten Gruppe A, 19 Patienten Gruppe B, 4 Patienten Gruppe C und einen Patienten Gruppe D. Im Rolimeter®-Test ergab sich beim Seitenvergleich ein durchschnittlicher Unterschied von 1,6 mm (0,5-3,5 mm) zur Gegenseite. Unsere Ein-Jahres-Ergebnisse sind gut, mittel- und langfristige Ergebnisse stehen bei dieser neuen Methode noch aus.

Reconstruction of the anterior cruciate ligament in the TransFix®-technique - one year results

Summary

The Endobutton-technic has been in discussion for the last couple of years because of the insufficient fixation near to the joints and the resulting instability of the anterior cruciate ligament. The substitute operation in which the middle part of the patellar ligament is used has been critically topicalized because of the high morbidity of harvest and because of problems concerning once more the fixation near to the joints. In this case, the reconstruction of the anterior-cruciate ligament in which the Hamstring tendons are used and a fixation applying the TransFix©-Pin sum up to an interruptible femoral fixation and to a near to the joint lying tibial fixation by using a biodegradible interference screw. By applying the described technique the reconstruction of the anterior cruciate ligament (TransFix©- technique), 148 patients were treated in Nuremberg's department of accident surgery during the period from the 01. 10. 1998 until the 30. 06. 2000. Out of these 148 patients, meanwhile 32 were given a second check up after one year had elapsed since the day of their operation. The IKDC Score judges 8 patients to be group A, 19 patients to be in group B and renders 4 patients to group C and only one patient to group D. The Rolimeter©-test shows an average difference of 1.6 mm (0.5 -3.5 mm) in respect to the opposite side. Our first annual scores therefore give an excellent prospect on the future of our new method although we obviously cannot provide any long term results yet.

Résumé

La réparation des ligaments croisés par la technique Endobutton est de plus en plus discutée ces dernières années en raison de la fixation insuffisante au voisinage de l'articulation et de l'instabilité résultante. Il en est de même de l'opération de remplacement par le tiers moyen du tendon rotulien qui est l'objet de discussions critiques croissantes en raison de la morbidité du lieu du prélèvement du transplant ainsi que de la fixation près de l'articulation. L'opération de plastie du ligament croisé avec l'utilisation des tendons ischio-jambiers et fixation fémorale résistante à l'arrachement par Trans-Fix Pin ainsi qu'une fixation para-articulaire tibiale à l'aide d'une vis interférente biodégradable représente une bonne alternative. La technique opératoire est au point. Du 1.10.1998 au 30.06.2000, 148 patients ont été opérés selon cette technique à la Clinique de Traumatologie de Nuremberg. Parmi les 148 patients 32 ont pu être contrôlés à 1 an. Selon le score IKDC 8 patients ont été côtés en groupe A, 19 en groupe B, 4 en groupe C et 1 en groupe D. En comparaison avec le groupe opposé, le Rollimeter-Test a montré une différence moyenne de 1,6 mm (0,5-3,5 mm). Nos résultats à 1 an sont bons; les résultats à moyen et long terme de cette méthode récente ne sont pas encore connus.

Resumen

La técnica del endo-botón ha sido discutida durante los dos últimos años debido a la insuficiente fijación y la resultante inestabilidad del ligamento cruzado anterior. La operación de sustitución mediante la que el tercio medio del ligamento patelar es usado ha sido críticamente considerada debido a la alta morbilidad de la zona de injerto y a los problemas relacionados con la fijación. En este caso, la reconstrucción del ligamento cruzado anterior en la que se usan como injerto los músculos de la pata de ganso y se fijan aplicando el Clavo TransFix sumado a una fijación femoral y tibial usando un tornillo de interferencia biodegradable. Aplicando la técnica descrita (Técnica TransFix) se trataron 148 pacientes en el Departamento de Traumatología de Nuremberg durante el período 01.10.98 al 30.06.2000. De estos pacientes, 32 fueron sometidos a un segundo chequeo un año después de la intervención. El Score IKDC determinó que 8 pacientes estaban en el grupo A, 19 en el B, 4 en el C y sólo 1 en el D. El test Rolimeter mostró una diferencia promedio de 1,6 mm (0,5-3,5 mm) con respecto al lado opuesto. Nuestro primer score anual en consecuencia dio una excelente perspectiva sobre el futuro de nuestro nuevo método aunque obviamente no puede prever resultados a largo plazo todavía.
