Zusammenfassung.

Dargestellt wird der Fall einer 27-jährigen I. Gravida, I. Para, bei der trotz regelmäßiger Schwangerschaftsvorsorgeuntersuchungen erst in der 33. SSW eine Knochenfehlbildung am Kind diagnostiziert wurde. Es handelte sich um die seltene Form eines beidseitigen Femur-Fibula-Ulna (FFU)-Komplexes. Der FFU-Komplex ist eine nicht letale Fehlbildung mit meist asymmetrisch auftretender Kombination aus Femur- und Fibuladefekten mit kontralateraler Ulnafehlbildung. Der FFU-Komplex lässt sich je nach Zahl der betroffenen Extremitäten in vier Gruppen klassifizieren. In nur ca. 10 % der Fälle findet sich ein Befall aller Extremitäten. Unser Fall (Typ IV) zeigt eine Peromelie beider oberer Extremitäten mit stumpfförmigem Enden beider Humeri, beidseitiger Fibulaaplasie und Aplasie des 4. und 5. Fußstrahls. Innere Fehlbildungen fanden wir nicht. Die Ätiologie der Fehlbildung ist unbekannt, das Auftreten sporadisch. Chromosomopathien wurden bislang nicht beobachtet. Ein familiär gehäuftes Auftreten wird nicht beschrieben. Das Alter der Eltern zeigt keine Zusammenhänge zum FFU-Komplex. Differenzialdiagnostisch abzugrenzen sind weitere Fehlbildungskomplexe mit Reduktionsanomalien der Extremitäten wie z. B. die Thalidomiembryopathie, die splenogonadale Fusion mit Extremitätenfehlbildung, das Roberts-Syndrom, der oroakrale Komplex oder die Acheiropodie. Abzugrenzen sind zudem Amnionstrangsequenzen.

We present a case of a 27 year old I. gravida, I. para. Despite of regular ultrasonograhpic examination the diagnosis of sceletal malformation at the fetus was not before 33. weeks of gestation. It was the rare type of a bothside femur-fibula-ulna (FFU) complex. The FFU-complex is a no lethal malformation with typical unilateral combination from defects of femur and fibula, with contralateral defect of ulna. Dependent to involment of malformated limbs the FFU-complex is classifiable in four groups. Only in 10 % all limbs are affected. Our case (type IV) showed a peromelia of both upper limbs with stumps of humeri, bothside aplasia of fibula and missing from 4. and 5. toes. There fetus were without nonlimb congenital abnormalities. The etiology of FFU-complex is unknown, the occurence sporadic. There are never found genetic abnormalities. Familial recurrence is not described. There is no maternal or paternal age effect on FFU-complex. The differential diagnosis must include malformations with reduction anomalies of extremities, like thalidomide syndrome, splenogonadal fusion with limb malformations, Roberts syndrome, oroacral complex or acheiropodia. Mark off are amniotic band too.
