Zusammenfassung
Die angeborene Zwerchfellhernie kann eine lebensbedrohliche Fehlbildung sein. Die Lungenhypoplasie, sowie die Verdrängung der Lunge und des Mediastinums durch den intrathorakal befindlichen Darm führen zur respiratorischen und kardiovaskulären Insuffizienz, sowie gelegentlich zur intestinalen Obstruktion durch eine Inkarzeration. Es wird ein sechs Monate alter Säugling mit Magenperforation beschrieben. Bei der Notoperation stellte sich eine linksseitige echte Zwerchfellhernie mit Inkarzeration des Magens dar. Die Perforation wurde zweireihig übernäht, das Zwerchfell primär verschlossen.
Summary
Diaphragmatic herniae presenting past the neonatal period can lead to life threatening emergencies. In these rare occurrencies patients may show acute respiratory and cardio-circulatory compromise or bowel obstruction due to incarcerated viscera. We report the case of a previously healthy 6 months old boy, with a sudden gastric perforation. At emergency laparotomy a left postero-lateral diaphragmatic hernia with an incarcerated and perforated stomach was found. The gastric perforation was repaired with a two layer and the diaphragmatic hernia with a primary closure.
