Entzündliche Erkrankungen des großen Netzes - eine schwierige präoperative Differenzialdiagnose

Zusammenfassung

Isolierte, entzündliche Veränderungen des großen Netzes sind seltene Erkrankungen, die präoperativ kaum differenzialdiagnostisch ins Kalkül gezogen werden und oft erst intraoperativ erkannt werden. Zwischen 1999 und 2001 wurde bei vier Patienten im Alter von 44-88 Jahren (2 männl.) eine Laparotomie wegen einer entzündlichen Raumforderung bzw. eines akuten Abdomens durchgeführt. Das C-reaktive Protein war in allen Fällen erhöht. Bei zwei Patienten fand sich eine primäre Torsion des großen Netzes, in den beiden weiteren Fällen eine paracolische pseudotumoröse Omentitis. Nach Resektion des pathologischen Netzanteiles war der postoperative Verlauf in allen Fällen komplikationslos. Torsionen des großen Netzes und Omentitiden sind mit abdominellen Beschwerden verbunden, die oft andere häufige Erkrankungen wie z. B. eine akute Appendicitis imitieren. Die partielle Omentektomie ist die Therapie der Wahl.

Abstract

Isolated inflammatory alterations of the greater omentum are rare diseases. Since the preoperative diagnosis is difficult these changes are usually ascertained upon laparotomy. Between 1999 and 2001 four patients (44-88-years-old, 2 males) underwent laparotomy for an inflammatory tumor mass or an acute abdomen, respectively. C-reactive protein was elevated in all cases. In two cases a primary torsion of the greater omentum was found, in the other two a paracolic pseudotumorous omentitis. Resection of the diseased omental parts led to complete recovery. Torsion of the greater omentum and omentitis cause abdominal symptoms with an inflammatory component that often mimics other more common diseases such as acute appendicitis and urges laparotomy. Partial omentectomy is the therapy of choice.
