Zusammenfassung

Einleitung: 5-10 % aller Shuntoperationen bei Hydrocephalus internus werden auch heute noch durch eine Infektion kompliziert. Peritonealer und kardialer Shunt sind mit etwa gleicher Häufigkeit betroffen. Die Behandlung infizierter Shuntsysteme erfolgt in der Regel durch eine intravenöse oder auch intrathekale Antibiotikatherapie, der Entfernung der Ableitung und der Neuanlage. Über den Zeitpunkt der Neuimplantation des Shunts existieren keine einheitlichen Empfehlungen.
Methode: 862 Fälle unseres Krankengutes der Jahre 1965-1999 wurden bezüglich der Infektionshäufigkeit retrospektiv beurteilt.
Ergebnisse: Zunächst war die Infektionsrate 10,1 %, ab 1980 4,4-5 %. Wir führen diese günstige Entwicklung auf ein subtiles operatives Management, eine perioperative Antibiotikaprophylaxe sowie die Einführung und den indikationsgerechten Einsatz der ventillosen ventrikulo-peritonealen Ableitung zurück.
Bei der Beurteilung des postoperativen Verlaufes nach infektionsbedingter Ableitungsentfernung bei 10 unserer Patienten benötigten 4 über einen Zeitraum von nunmehr 1 bis 7 Jahren infolge fehlender Hirndruckentwicklung keine erneute Drainageoperation. Die Krankheitsverläufe dieser 4 Patienten werden demonstriert.
Diskussion: Ausgehend von einer auch retrospektiv stimmigen Indikation zur Ableitungsoperation in jedem der 4 Fälle und unterstellend, dass im Lebensalter von zumindest 3 der 4 Kinder ein Arrestieren des Hydrocephalus nicht zu erwarten ist, bleibt eine schlüssige Erklärung letztlich unklar. Das Procedere eines relativ kurzfristigen Systemwechsels ggf. mit intermittierender externer Drainage nimmt dem Chirurgen die Möglichkeit der Erkennung des Vorliegens oder der Entwicklung eines kompensierten Hydrocephalus und dem Kind die Chance der Erreichung eventueller Shuntunabhängigkeit.
Schlussfolgerung: Nach infektionsbedingter Ableitungsentfernung muss vor der Platzierung eines neuen Shuntsystems eine erneute Überprüfung der Shuntpflichtigkeit empfohlen werden.

Abstract

Introduction: Postoperative infections occur as complications in 5-10 % of all cases after CSF shunting procedures either with peritoneal or cardial shunts. Generally, infected shunts were treated with intravenous or intrathecal antibiotics accompanied by the removal of the shunt and subsequent replacement. There is controversy about the timing of the reshunting.
Methods: In a retrospective study, 862 patients with internal hydrocephalus who received a first shunt in the years from 1965-1999 were investigated with special reference to infection rate, treatment and outcome.
Results: The average infection rate for all implanted CSF shunts was 10.1 percent until 1980 and decreased from 1980 to 1999 to 4.4-5 percent. The main reasons for this advantageous development are seen in an improved subtile surgical technique, perioperative antibiotic prophylaxis and the implementation of the ventriculoperitoneal shunting without valve in strictly indicated cases of very low birth weight newborns.
Within the last decade, 10 patients underwent removal of the CSF-shunt due to an infection. 4 of those children did not require a new shunt. During the follow up time of 1 to 7 years they remained free of symptoms of an increased intracranial pressure. These 4 cases are presented in this paper.
Discussion: Retrospectively, the indication for primary CSF-shunting was found to be correct in all 4 patients. In 3 of 4 patients, development of an arrested hydrocephalus was unlikely due the age at removal of the infected shunt. Therefore, an explanation of the observation that reshunting was not necessary in those cases could not be found. The standard-procedure of early reshunting with and without intermittent external drainage gives the surgeon no opportunity to detect the possible development of an arrested hydrocephalus which makes the patient independent of shunting.
Conclusion: After infection related removal of a shunt system a critical reappraisal of shunt-dependence is recommended before reshunting.
