Zusammenfassung

Seit der Einführung retardierter Opioide in die Schmerztherapie vor mehr als 20 Jahren, sind weitere Anstrengungen zur Verbesserung der Patientenversorgung unternommen worden. Doch trotz der Entwicklung effektiver und einfach anzuwendender Behandlungsrichtlinien durch die WHO leiden immer noch zu viele Krebspatienten unnötigerweise an Schmerzen oder den Nebenwirkungen der Therapie. Für die meisten Patienten besteht eine zufrieden stellende medikamentöse Einstellung nach einer Periode der Dosisanpassung in einem akzeptablen Gleichgewicht zwischen Wirkung und Nebenwirkung. Wünschenswerte Eigenschaften neuer Medikamente und Methoden sind oft nicht stärkere Wirkeffekte, sondern reduzierte Nebenwirkungen, da letztere die Ursache für einen Therapieabbruch oder die Begrenzung auf niedrige und damit unwirksame Dosierungen darstellen. Ausgehend von neuen Entwicklungen tumorspezifischer Therapieverfahren werden beispielhaft viel versprechende Neueinführungen von Medikamenten und aktuelle wissenschaftliche Erkenntnisse über „alte Pharmaka” diskutiert.

Abstract

Since the implementation of slow release opioids some 20 years ago in pain management, further efforts towards improvement of pain therapy were undertaken. However, despite the development of simple and effective guidelines by the WHO to treat cancer pain, too many cancer patients still suffer unnecessarily from pain or the side effects of the therapy. For most patients, a satisfactory outcome following an initial analgesic titration period results in an acceptable balance between analgesia and side effects. Preferable pharmacological characteristics of new drugs and methods are not only stronger analgesic effects, but reduced side effects, since the latter are responsible for withdrawal of treatment on low and ineffective dosages. New developments of antineoplastic therapies are discussed as well as promising new drugs and current scientific insights in „old drugs”.
