Metagedächtnisleistungen bei Alzheimerpatienten

Zusammenfassung

38 Alzheimerpatienten gaben eine qualitative (persönliche Einschätzung der Veränderung der eigenen Gedächtnisleistung) sowie eine quantitative Beurteilung ihrer eigenen Gedächtnisleistungen (Leistungsvorhersage bei einer Gedächtnisaufgabe) und der ihrer Bezugspersonen ab. Die qualitative (globale) Gedächtnisbeurteilung erwies sich als unabhängig von aktuellen Leistungsdefiziten, während die Genauigkeit quantitativer Leistungsvorhersagen bezogen auf die eigene Gedächtnisleistung mit zunehmenden kognitiven Beeinträchtigungen abnahm. Kognitive Beeinträchtigungen der Demenzpatienten hatten bei der Prädiktion der Leistung ihrer Angehörigen dabei keinen Effekt. Beide Aspekte der Metakognition, sowohl die qualitative als auch quantitative Einschätzung eigener Gedächtnisleistungen, zeigten einen Zusammenhang mit prämorbiden Bewältigungsstrategien. Patienten, denen zugeschrieben wurde, vermehrt Strategien der kognitiven Vermeidung anzuwenden, gaben häufiger an, dass ihre Gedächtnisleistungen sich nicht wesentlich verändert hätten bzw. überschätzten ihre Gedächtnisleistungen.
Die vorliegenden Ergebnisse verdeutlichen dabei die Methodenabhängigkeit der Ergebnisse zum Zusammenhang metakognitiver Leistungen und dem Schweregrad der Demenz und machen darüber hinaus deutlich, dass der Wahrnehmung eigener Defizite ein komplexes, multi-dimensionales Konzept zugrunde gelegt werden muss.

Abstract

Thirty-eight patients with Alzheimer's disease were asked to give a qualitative (estimation of memory changes) and quantitative assessment of their own performance on memory tasks and also of their relatives' performance. Qualitative assessment showed to be independent from measured memory-deficits, while the precision of quantitative prediction of performance deteriorated with increased cognitive impairment. Cognitive impairment of the demented patients did not influence the prediction of the performance of their relatives. Both, qualitative and quantitative assessment of memory performance were influenced by premorbid coping-strategies. Patients, who were attributed to use strategies of cognitive avoidance, more often claimed to have no changes in memory function or over-estimated their memory performance. The results of this investigation reveal that methodological issues influence studies on the association of meta-cognitive abilities with the severity of dementia to a great extent. They also suggest that awareness of deficits should be based on a complex, multi-dimensional concept.
