1. Epidemiologie

Die Zwangsstörung ist eine chronische, die Lebensqualität stark einschränkende Erkrankung, die mit einer Lebenszeitprävalenz von 2 % in der erwachsenen Allgemeinbevölkerung wesentlich häufiger als früher angenommen vorkommt [1] [2]. Die Störung beginnt in vielen Fällen bereits in der Kindheit, der Häufigkeitsgipfel der Erstmanifestation liegt im späten Jugendalter. In den ersten Jahren der Erkrankung überwiegt ein episodischer Verlauf mit wechselnder Intensität der Symptomatik, der später häufig in einen chronischen Verlauf einmündet. Mit einer Spontanremission kann in der Regel nicht gerechnet werden. Ca. 50 % der Patienten haben die Erkrankung praktisch ihr gesamtes Leben lang [3].

Die Patienten neigen sehr stark zur Verheimlichung ihrer Symptome, derer sie sich meist schämen, so dass meist viele Jahre vergehen, bis eine korrekte Diagnose gestellt und eine Behandlung eingeleitet wird. Die Erkrankung wird deshalb auch als die „heimliche Krankheit” bezeichnet. Es ist davon auszugehen, dass die Mehrzahl der Patienten im Laufe ihres Lebens keine, zumindest keine effektive, störungsspezifische Behandlung erhält. Darüber hinaus ist die Versorgungssituation durch einen Mangel an spezialisierten Therapeuten gekennzeichnet, die mit der schwierigen und aufwändigen Therapie der Zwangsstörungen ausreichend vertraut sind.

Die Zwangsstörung ist wesentlich häufiger als vielfach angenommen. Es besteht eine sehr hohe Verheimlichungstendenz. Viele Patienten sind jahrelang wegen depressiver Episoden oder anderer psychischer Störungen in Behandlung. Ein gezieltes Nachfragen nach Zwangssymptomen mit 3 Screeningfragen sollte bei der Anamneseerhebung jeder psychischen Erkrankung nicht fehlen.
