Zusammenfassung

Während akuter Schmerz eine Warnfunktion erfüllt und ein Korrektiv in Reifung und Entwicklung darstellt, kommt rezidivierenden Schmerzsyndromen wie Migräne oder Clusterkopfschmerz und chronischen Schmerzzuständen eine eigenständige Krankheitsbedeutung zu.

Definitionsgemäß wird dabei die unterbewusste Nozizeption prinzipiell von dem Bewusstseinsphänomen Schmerz unterschieden. Das nozizeptive System ist in der Peripherie, an den synaptischen Verbindungen, in Rückenmark und Gehirn dynamisch. In den 1990er Jahren wurde erkannt, dass auch Glia aktiviert werden kann.

Adipositas ist ein Risikofaktor für die Entwicklung von Rückenschmerzen und von Gelenkschmerzen der unteren Körperhälfte. Mit der Gewichtsverminderung gehen die entsprechenden Schmerzsyndrome im Mittel zurück oder sistieren. Die wichtigsten Prädiktoren des Übergangs akuter in chronische Schmerzen sind daneben die heftige Intensität akuter Schmerzen, der Verarbeitungsstil, depressive Störungen, Angsterkrankungen, Substanzmissbrauch und sozioökonomische Faktoren.

Abstract

Acute pain has a warning function and is a regulative in human maturation. In contrast, recurrent pain syndromes like migraine or cluster headache and chronic pain states are diseases in their own right.

By definition, somatic nociception has to be differentiated from pain as a phenomenon of consciousness. The nociceptive system is dynamic in the periphery, at the synaptic connections, in the spinal cord, and brain. In the 90th’s, it was discovered that glia can be activated.

Obesity is a risk factor for the development of back pain and of arthralgia of the lower half of the body. In the case of weight decrease of obese patients, the correlating pain syndromes ameliorate or disappear completely. The most important predictors of a transition from acute to chronic pain states are a severe intensity of acute pain, the coping style, depressions, anxiety disorders, substance abuse and socio-economic factors.
