Statusbericht: Intraligamentale Terminalanästhesie

Zusammenfassung

In ihrem Statusbericht zur intraligamentären Anästhesie (ILA) befand die American Dental Association 1983, dass bei dieser Technik eine ausgedehnte praktische Einübung erforderlich und nur ein geringer Spielraum für Fehler gegeben sei. Empfohlen wurde die ILA nur als Zusatzanästhesie bei unzureichender Pulpaanästhesie, bei indizierten endodontischen Behandlungen, für Kinder und bei einer Patientenpräferenz. Offen waren die Fragen zu den histologischen Wirkungen auf und die Ausbreitung der Anästhesielösung durch das periodontale Ligament und die benachbarten Strukturen sowie die Mechanismen der Anästhesiewirkung und die Effekte dieser Injektion auf das Pulpengewebe. Durch evidenzgestützte Studien wurden alle damals gestellten Fragen vollständig aufgeklärt. Die intraligamentale Anästhesie ist den anderen Methoden der Lokalanästhesie in allen Aspekten zumindest ebenbürtig, zum Teil sogar signifikant überlegen, wenn sensible Instrumente verwendet, die Technik lege artis praktiziert und ihre Durchführung beherrscht werden.

Summary

In 1983 the American Dental Association postulated that with this technique seemed to be little margin for error and that extensive practice is necessary for development of the manual skills needed for consistently favorable results. It was recommended that the periodontal ligament injection should be used only as an adjunct to conventional injection techniques in case conventional blocks have produced inadequate pulpal anesthesia, in children, in the management of endodontically involved teeth and in cases of patient preference. Additional research needed to be done to determine the histologic effects on the periodontal ligament produced by the periodontal ligament injection and the spread of the anesthetic solution through the periodontal ligament and adjacent structures, as well as the mechanism of anesthetic action. The effects of this injection on the dental pulp tissue was also to be examined. Today - 20 years later - it can be stated that all postulated questions have been completely cleared up by evidence based studies. It could be shown that the intraligamental single tooth anesthesia is at least equivalent to the other techniques of local anesthesia even superior in case sensible instruments are used and the anesthesia method is practised lege artis and mastered safely by the operator.
