Langzeitergebnisse der operativen Behandlung des vertebrobasilären Syndroms

Zusammenfassung

Das operatives Vorgehen stellt eine etablierte Behandlungs-Methode bei Läsionen im Bereich der extrakraniellen Karotisabschnitte dar. Die Veränderungen im vertebrobasilären Stromgebiet können ebenfalls ein Durchblutungsdefizit hervorrufen, das nicht nur durch Arteriosklerose sondern auch durch eine Kompression der Vertebralarterien im Wirbelkanal enstehen kann. Im vorliegenden Bericht werden 54 Patienten analysiert, die zwischen 1994 und 2003 wegen einer Kompression der A. vertebralis operativ behandelt wurden. Eine distale End-zu-Seit-Anastomose der A. vertebralis mit der A. carotis externa wurde durchgeführt. Bei der Mehrzahl der Patienten konnte ein gutes oder sehr gutes klinisches Ergebnis erzielt werden.

Abstract

In case of changes occurring within the extracranial sections of the common and internal carotid artery the operative procedure has become a generally accepted surgical method. Another reason of brain ischemia is the vertebro-basilar insufficiency caused by external pressure on the vertebral artery in a channel formed by transverse processes of cervical vertebrae. The author presents the clinical experience with 54 cases that have been treated surgically from 1994 to 2003 due to the above mentioned reasons. In all patients an anastomosis between the distal vertebral artery and external carotid artery gave good or very good results.
