Zusammenfassung

Kohlenhydratarme Diäten eignen sich zur kurzfristigen Gewichtsreduktion. Das gilt besonders für kohlenhydrat- und fettarme vollbilanzierte Formula-Diäten. Derzeit gibt es jedoch keine kontrollierten Studien, die zeigen, dass kohlenhydratarme Diäten auch eine erfolgreiche Stabilisierung des Gewichts über Jahre ermöglichen. Eine deutlich höhere Evidenz spricht dafür, dass fettarme Diäten (25-30 Energie% Fett) ohne Beschränkung der Kohlenhydrataufnahme eine gute diätetische Strategie zur Gewichtsstabilisierung sind. Bei mittel- bis langfristiger Anwendung von kohlenhydratarmen Diäten sind Gesundheitsrisiken mangels fehlender Studien ebenfalls nicht auszuschließen. Diäten mit verringerter glykämischer Ladung haben für Stoffwechselgesunde keinen zusätzlichen Nutzen für das Gewichtsmanagement, profitieren können jedoch Typ-2-Diabetiker und Personen mit gestörter Glucosetoleranz.

Summary

Low-carb diets are suitable for short-term weight reduction. This particularly applies to fully balanced formula low-fat and low-carb diets. Unfortunately, controlled studies proving successful weight stabilization for several years are still lacking. There is a significantly higher evidence that low-fat diets (i. e. 25 to 30 per cent of daily energy intake come from nutritional fat) without a limitation of carbohydrate ingredients are a good strategy for weight stabilization. Owing to the lack of pertinent studies, a medium- and long-term application of low-carb diets might even have a detrimental effect on health, e. g. changes in the lipid pattern, kidney stones, osteoporosis, hyperhomocysteinaemia. Whereas people not affected by metabolic anomalies will not have an extra benefit from a diet with reduced glycaemic load, type-2 diabetics and persons with impaired glucose tolerance could profit from such a diet.
