Zusammenfassung
Die diabetische Neuropathie ist definiert als eine Dysfunktion des peripheren Nerven
bei Diabetikern nach Ausschluss anderer Ursachen. Bereits heute die häufigste Form
einer Polyneuropathie in der westlichen Welt, wird ihre Prävalenz aufgrund der demografischen
Entwicklung weiter zunehmen. Die unterschiedlichen Manifestationsformen verlangen
ein abgestuftes diagnostisches Programm. Es ist gezielt nach Symptomen zu fahnden,
die für eine andere Genese der Neuropathie sprechen. Ein besondere differenzialdiagnostische
Herausforderung ist die Abgrenzung von kausal behandelbaren, möglicherweise koexistenten
dysimmunen/entzündlichen Neuropathien.
Summary
Diabetic neuropathy is defined as the presence of symptoms and/or signs of peripheral
nerve dysfunction in people with diabetes after exclusion of other causes. Diabetic
polyneuropathy is the most common peripheral neuropathy in the western world, and
demographic changes make prevalence further rise. Clinical manifestations of diabetic
neuropathy are heterogeneous. In the assessment of neuropathy, „red flags” pointing
to non diabetic causes of neuropathy should prompt further diagnostics. Coexistent
dysimmune/inflammatory neuropathies are challenging in differential diagnosis but
causatively treatable conditions.
