Zusammenfassung
Ein junger Mann mit einer schweren paranoid-halluzinatorischen Psychose stürzte sich
im zweiten Jahr der stationären Behandlung zweimal in 6 Monaten in suizidaler Absicht
aus Gebäuden. Erst Clozapin führte zu einer Stabilisierung des klinischen Bildes. Dieser Behandlungsverlauf und
die diskutierte Literatur zeigen, dass die für schizophrene Patienten typischen schweren
Suizidhandlungen häufig multifaktoriell bedingt sind. Das klinische Management muss
daher bei dieser Patientengruppe die psychiatrisch-psychotherapeutischen Aspekte der
Suizidprävention beachten, wobei Clozapin aus suizidpräventiver Sicht einen besonderen Stellenwert hat.
Abstract
This report documents a young male inpatient, whose severe schizophrenic disorder
was treated unsatisfactorily by using several conventional and atypical anti-psychotic
medications. During hospitalisation over a period of six months, the young man tried
to commit suicide twice - each time by jumping out of a tall building. Finally, only
treatment with clozapine proved effective in bringing about sufficient stabilization.
In accordance with the literature available on similar cases, this particular case
shows a multi-factored background of sudden severe suicidal behaviour typical for
patients suffering from severe schizophrenia. Thus clinical treatment for such patients
has to take specific aspects of suicide prevention into consideration, in addition
to the antipsychotic treatments already in use. The use of clozapine might be of significant
importance in such cases.
