Akute vorübergehende psychotische Störungen - Eine Frauenkrankheit?

Die ICD-10-Kategorie F23 der „akuten vorübergehenden Psychosen” reflektiert den Versuch, eine Gruppe von Psychosen zu erfassen, die nicht identisch mit der Schizophrenie oder den schizoaffektiven Psychosen sind. Es handelt sich um turbulente, manchmal dramatisch aussehende Psychosen, die vorwiegend das weibliche Geschlecht betreffen. Sie haben einen akuten oder eben auch abrupten Ausbruch. Ihre Manifestation ist nur selten mit einer akuten Belastungssituation verbunden. Sie reagieren gut auf antipsychotische Medikation und viele davon auch auf eine Prophylaxe mit Affektstabilisatoren wie etwa Lithium. Trotz häufiger Rezidive ist die Langzeitprognose meist gut. Dies rechtfertigt die klinische Abgrenzung dieser Störungen von der Schizophrenie.

The ICD-10 category F23 „Acute and Transient Psychotic Disorders” reflects an attempt to describe a group of psychosis neither identical with schizophrenia nor with schizoaffective disorders. Typically, the clinical picture is turbulent, sometimes dramatic. Primarily, women are affected. The onset is acute or even abrupt. A preceding severe psychosocial stressor is rare. Therapeutic response to antipsychotics is good. In some cases mood stabilizer such as lithium are indicated. Although relapse is frequent, long-term prognosis is mostly favourable, therefore the clinical distinction of this disorder from schizophrenia is important.
