Bereits 1937 unterschied Fishberg [6] in seinem Lehrbuch Heart Failure zwei Formen der Herzinsuffizienz: „.those forms of cardiac insufficiency which are due to inadequate diastolic filling of the heart..(and) the far more common ones in which the heart fills adequately but does not empty to the normal extent”. Beide Formen der Herzinsuffizienz, diastolisch und systolisch, bewirken eine reduzierte Auswurfleistung mit dem Leitsymptom Belastungsdyspnoe. Es bestehen jedoch fundamentale Unterschiede nicht nur in der Pathophysiologie, sondern es handelt sich um zwei unterschiedliche Syndrome mit eigener Epidemiologie, Diagnostik, klinischen Verlauf und auch Therapie [22] [23].
