Sekundärglaukom als „Masquerade” eines kutanen Melanoms mit Metastasierung in den Glaskörperraum und in die vordere Augenkammer

Einleitung

Intraokulare Metastasen eines kutanen Melanoms sind selten, am häufigsten metastasieren sie in die Aderhaut [5]. Die Metastasierungen in die Netzhaut und besonders in den Glaskörperraum sind schwer einzuordnen. Die Metastasierung in den Glaskörperraum manifestiert sich entweder als vereinzelte Zellen oder Zellenaggregate, die meistens in Schichten oder in linearen Fäden angeordnet sind [5] [6] [9]. Die Blutung im Glaskörperraum und ein Neovaskularisationsglaukom (NGV), die beide zur Funktionseinschränkung führen, sind nicht nur die häufigsten, sondern auch die schwerwiegendsten Komplikationen solcher Metastasierung [10]. Wir berichten über den Fall eines kutanen Melanoms mit Metastasierung in den Glaskörperraum und in die vordere Augenkammer, was sich klinisch als ein Neovaskularisationsglaukom manifestierte. Bevor wir die Diagnose eines intraokularen Melanoms stellen konnten, wurde die Phakoemulsifikation mit Implantation einer Intraokularlinse durchgeführt und die Glaukomdränage platziert.
