Zusammenfassung

Ziel der Studie Für die Behandlung subretinaler zentraler Neovaskularisationsmembranen im Rahmen einer AMD wird seit mehreren Jahren die Radiotherapie diskutiert. Ziel dieser Studie war es, die Wirksamkeit dieser Therapie für okkulte bzw. klassische CNV zu prüfen und die Resultate einer hochdosierten mit jenen einer niedrigdosierten Radiotherapie (Rx) prospektiv und kontrolliert zu vergleichen.

Methode und Patienten Eingeschlossen wurden Patienten mit einem Alter von 60 bis 85 Jahren, sowie einem Visus von 0,1 bis 0,6 mit entweder subfovealer/juxtafovealer okkulter oder klassischer CNV. Die Strahlendosis betrug 2 Gy 5 × wöchentlich bis zu einer Gesamtdosis von 10 Gy bzw. 36 Gy. Für 95 Augen liegt eine Nachbeobachtungszeit von 12 - 24 Monaten vor.

Ergebnisse 8 Augen mit okkulter CNV erhielten 36 Gy, 16 Augen wurden mit 10 Gy bestrahlt und 21 Augen waren in der Kontrollgruppe. Generell zeigten sich keine Unterschiede im Visusverlauf zwischen hoher oder niedriger Strahlendosis. Der durchschnittliche Visusverlust betrug nach 12 Monaten in allen Gruppen 3,5 Zeilen und nach 24 Monaten 5 Zeilen. Bei den klassischen CNV wurden ebenfalls 8 Augen mit 36 Gy bestrahlt, 27 Augen erhielten 10 Gy und 15 Augen gehörten zur Kontrollgruppe. Der mittlere Linienverlust betrug nach 6 Monaten dosisunabhängig für die bestrahlten Augen 2,2 Zeilen und für die nichtbestrahlten Augen 5,7 Zeilen, was signifikant war (p < 0,05). Nach 12 (24) Monaten hatten 50 (75 %) mit 36 Gy, 48 (83) % mit 10 Gy und 60 (83) % in der Ko-Gruppe einen Visus von < 0,71, womit sich eine Signifikanz nach 12, aber nicht nach 24 Monaten ergab.

Diskussion und Schlussfolgerungen Der natürliche Verlauf okkulter CNV konnte durch Rx mit 10 oder 36 Gy nicht beeinflusst werden. Bei den klassischen CNV konnte mit Rx häufiger ein Fernvisus von 0,1 und mehr als ohne Rx für mindestens 12 Monate erhalten werden. Nach 24 Monaten zeigte sich kein Unterschied mehr. Die Bestrahlung mit 36 Gy wies, verglichen mit 10 Gy, keinen besseren Therapieeffekt auf und ist auch wegen Strahlenretinopathie in 31 % nicht vertretbar.

Aim of the study Several pilot studies have indicated, that radiation therapy might have a beneficial effect on the course of CNV in AMD. This controlled study was aimed at the question, whether such treatment might halt progression of neovascular AMD and whether a low or a high radiation dose should be applied.

Methods and patients Enclosed were patients aged ≥ 60 and ≤ 85 and eyes with a VA of ≥ 0.1 and ≤ 0.6, revealing a juxta/subfoveal CNV either of the occult or the classic type. Treatment was performed with a linear accelerator at fractions of 2 Gy up to a total dose of 10 Gy or 36 Gy. 95 eyes had completed follow-up of ≥ 12 ≤ 24 months.

Results Among eyes with occult CNV 8 received 36 Gy, 16 were treated with 10 Gy and 21 were in the control group. Mean visual loss was 3.5 lines after 12 months and 5 lines after 24 months with no difference between irradiated eyes and those in the control group. In the groups with classic CNV 8 eyes were treated with 36 Gy, 27 eyes received 10 Gy and 15 eyes were in the control group. Mean visual loss after 6 months was 2.2 lines in eyes of both groups treated with radiation and 5.7 lines in the control group. This was statistically significant (p < 0.05). VA was < 0.1 after 12 (24) months of follow-up in 50 (75) % of the cases with 36 Gy, in 48 (83) % with 10 Gy and in 60 (83) % of the controls. These results were statistically significant after 12 months only.

Discussion and conclusions The natural course of occult CNV could not be improved by irradiation with 10 or 36 Gy. In eyes with classic CNV a VA of ≥ 0.1 was maintained significantly more often in irradiated eyes than in those of the control group. Treatment with 36 Gy however was associated with an unacceptable incidence of radiation retinopathy.
