Die Hypospadie beim Mädchen

Abstract

Female hypospadias is a rare, but well defined anomaly of the lower urogenital system. From an embryologic point of view it represents an arrest during a late stage of sinus urogenital development. It is not to be confused with minor variants of the external urethral meatus of a doubtful clinical relevance. The main symptom is a severe subvesical obstruction, and usually it presents early in infancy. A case is presented, in which on the basis of the incorrect diagnosis of "occult neurogenic bladder outlet obstruction" a supravesical diversion (ileal conduit) was done. Undiversion was successfully carried out at an age of 7 years.

Zusammenfassung

Die weibliche Hypospadie ist nicht unter die ,,häufigen Meatusanomalien" fraglicher klinischer Relevanz zu subsummieren. Vielmehr ist sie eine definierte Anomalie, die formalgenetisch in die Gruppe der Entwicklungsstörungen des Sinus urogenitalis gehört. Ihr hervorstechendes klinisches Symptom ist die subvesikale Obstruktion, die Manifestation erfolgt in der frühen Kindheit. Wahrscheinlich ist die Anomalie ausgesprochen selten. Es wird ein Fall beschrieben, in welchem die fälschliche Diagnose einer ,,okkulten neurogenen Blasenentleerungsstörung" zur supravesikalen Harnableitung in Form eines Ileum-Konduits geführt hatte. Im Alter von 7 Jahren konnte die Harnableitung erfolgreich wieder beseitigt werden.
