Zusammenfassung

Hintergrund: Die Versorgung von Patienten mit Nachweis von multiresistenten Erregern in operativen Einrichtungen ist Routine in deutschen Kliniken. Trotzdem besteht zum Teil Unsicherheit, welche Maßnahmen zur Prävention von Erregerübertragungen im OP zu ergreifen sind. Dies liegt unter anderem daran, dass spezifische Leitlinien nicht verfügbar sind. Material und Methoden: Die aktuelle Praxis in operativen Abteilungen ausgewählter universitärer Zentren in Deutschland wurde mittels Fragebogen erhoben. Zusätzlich wurden die Zentren gebeten, ihre internen Standards bezüglich Patienten mit MRE-Nachweis im OP-Bereich zu übermitteln, sofern verfügbar. Ergebnisse: Neunzehn Fragebögen aus 19 Abteilungen in 4 Zentren sowie 5 Standards aus 3 Zentren wurden in die Auswertung einbezogen. Es zeigte sich eine große Bandbreite der ergriffenen Präven­tions­maßnahmen sowohl zwischen den Zentren als auch zwischen einzelnen Abteilungen, auch bei vorhandenen internen Standards. Schlussfolgerung: Leitlinien zu MRE-positiven Patienten in operativen Abteilungen sollten praxisorientiert erarbeitet werden und sowohl Übertragungsrisiken als auch Überreaktionen be­grenzen. Verbesserungspotenzial besteht bei der Umsetzung interner Standards. 

Abstract

Background: Providing surgical treatment for patients colonised or infected with multidrug resistant organisms (MDROs) is daily routine in German hospitals. However, there is uncertainty about the application of adequate infection control measures in the OR. One of the reasons is that specific guidelines are not available. Material and Methods: We evaluated current practice in surgical departments of selected German university medical centres using a questionnaire. In addition, centres were asked to provide in-house standard operating procedures (SOP), if available. Results: Nineteen questionnaires from 19 departments within 4 centres and 5 in-house SOPs were ana­lysed. The results showed a broad spectrum of applied infection control measures. Wide variations existed both within centres and within departments of the same centre regardless of existing in-house standards. Conclusions: Guidelines addressing perioperative infection control measures for patients har­bour­ing MDROs should be developed with a focus on practicability to reduce both transmission of MDROs and unreasonable measures. Implementation of existing SOPs can be a target for optimisation.
