Zusammenfassung

Hintergrund Multiple, bilaterale Zysten der Pars plicata corporis ciliaris sind ein sehr seltener Befund. Die Abgrenzung von soliden, vom Ziliarkörper ausgehenden Prozessen kann schwierig sein.

Patientin Eine 23-jährige, sonst gesunde Patientin erlitt bei der augenärztlichen Untersuchung in Mydriasis einen akuten Winkelblockglaukomanfall. Der intraokulare Druck betrug beidseits 14 mm Hg bei spielender Pupille und stieg nach medikamentöser Mydriasis auf 26 mm Hg rechts und 44 mm Hg links an.

Ergebnisse Gonioskopisch zeigten sich an beiden Augen über die gesamte Zirkumferenz der Pars plicata multiple Zysten des pigmentierten und nicht pigmentierten Ziliarkörperepithels. Die Untersuchung mit dem Ultraschall-Biomikroskop zeigte dünne Zystenwände mit einem Lumen ohne Reflektivität. Am rechten Auge war das Irispigmentblatt gespalten. Der Kammerwinkel war in Miosis offen. Die Iriswurzel wurde durch multiple ca. 2 mm große Ziliarkörperzysten verlagert, so dass in Mydriasis der Kammerwinkel zirkulär verschlossen wurde. Beidseitige Nd:YAG-Laser-Iridotomien waren wirkungslos. Mit Pilocarpin 0,5 % AT 2 × täglich wurde Anfallsfreiheit erreicht, der intraokulare Druck blieb unter 19 mm Hg.

Schlussfolgerung Zysten des Ziliarkörpers können über eine mechanische Verlegung des Kammerwinkels zu einem Winkelblockglaukom führen. Mögliche therapeutische Maßnahmen sind 1.) Applikation von Miotika, 2.) Eröffnung pigmentierter Zysten mittels Argon-Laser-Koagulation bzw. nicht pigmentierter Zysten mit dem Nd:YAG-Laser, falls keine vorderen Synechien bestehen, 3.) filtrierende Operation. Vor einer Lasertherapie muss unbedingt eine zystische Epithelinvasion ausgeschlossen werden, da in dieser Situation eine vollständige Entfernung durch Blockexzision angestrebt werden sollte, um nicht durch eine Laserkoagulation eine diffuse Variante der zystischen Epithelinvasion mit drastisch verschlechterter Prognose zu induzieren. Neben der Gonioskopie kann die Ultraschall-Biomikroskopie (UBM) einen wichtigen Beitrag zum Ausschluss solider Ziliarkörperprozesse leisten.

Background Multiple bilateral cysts of the ciliary body corona are rare. The differentiation from solid tumors of the ciliary body is sometimes difficult.

Patient A healthy 23-year-old woman developed acute angle-closure glaucoma in mydriasis during ophthalmological examination. The intraocular pressure was 14 mm Hg in both eyes with normal pupils, 26 mm Hg in the right and 44 mm Hg in the left eye following pupillary dilation.

Results On gonioscopy there were multiple cysts of the pigmented and nonpigmented ciliary epithelium in both eyes. Ultrasound biomicroscopy disclosed these to be thin-walled cysts with no internal reflectivity. In the right eye the epithelium of the iris appeared separated. The chamber angle was open in miosis. The iris base shifted anteriorly secondary to multiple 2 mm large ciliary body cysts, causing circular angle closure in mydriasis. Bilateral Nd:YAG-iridotomy was ineffective. Treatment with pilocarpine 0,5 % normalized the intraocular pressure in both eyes.

Conclusions Cysts of the ciliary body can lead to mechanical angle closure. Possible treatment options include 1.) application of miotics, 2.) the opening of pigmented cysts by argon laser coagulation of pigmented and Nd:YAG laser coagulation of nonpigmented cysts, as long as there are no persisting anterior synechiae 3.) filtrating surgery. Cystic epithelial ingrowth has to be ruled out prior to laser treatment as in case of cystic epithelial invasion, a complete removal by en-bloque resection should be achieved. Cystic epithelial ingrowth may be transformed by laser coagulation into diffuse ingrowth with a worse prognosis and, ultimately, loss of the eye. It is possible to differentiate cysts from solid tumors of the ciliary body by means of ultrasound-biomicroscopy.
