Immunonutrition und postoperative Komplikationen unter G-DRGs

Zusammenfassung

Hintergrund: Ziel dieser Studie war die ökonomische Evaluation eines perioperativen Einsatzes von Immunernährung unter G-DRG-Bedingungen bei Patienten mit großen gastrointestinalen Eingriffen. Material und Methodik: Als Basis der Evaluation wurden klinische und ökonomische Daten von 2 prospektiven, randomisierten, doppelblinden, plazebokontrollierten Multizenterstudien von Senkal et al. aus den Jahren 1995 und 1998 verwendet. Erfasst wurden die direkten Leistungen für Diagnose und Therapie der postoperativen Komplikationen während des Krankenhausaufenthalts, die mit aktuellen (Jahr 2002) Krankenhauseinkaufspreisen oder mithilfe des DKG-NT-Tarifs bewertet wurden. Indirekte Kosten (z. B. wegen Arbeitsunfähigkeit) wurden nicht mit einbezogen. Die Komplikationskosten wurden mit den auf G-DRG (2004) basierenden Vergütungen verglichen und die Unterschiede zwischen dem Einsatz von Immun- respektive von Kontrollernährung evaluiert. Ergebnisse: Bei 69 von insgesamt 308 Patienten traten postoperative Komplikationen auf. In der Immunernährungsgruppe betraf dies 27 von 155 Patienten und in der Kontrollgruppe 42 von 153 Patienten (p = 0,012). Die Gesamtkosten (inkl. Ernährung) für die Behandlung der aufgetretenen Komplikationen pro Patient beliefen sich durchschnittlich auf 563 vs. 979 Euro (€) (Immunernährung vs. Kontrollernährung). Dies ergab eine wesentlich bessere Kosteneffektivität von 681 € für die Immunernährungsgruppe gegenüber 1350 € für die Kontrollgruppe. Die komplikationsbedingten Kosten pro Gruppe lagen total bei 67 438 € in der Immunernährungsgruppe verglichen mit 147 076 € in der Kontrollgruppe, inklusive Ernährung bei 87 212 vs. 149 837 €. Die Vergütung der Komplikationen basierend auf DRG wurde mit 178 084 € in der Immunernährungsgruppe und 277 259 € in der Kontrollgruppe berechnet. Für jeden Patienten mit Komplikationen resultiert ein Erlös von 3366 € in der Immunonutritionsgruppe und von 3034 € in der Kontrollgruppe. Schlussfolgerung: Auch unter DRG-Bedingungen ist der perioperative Einsatz von Immunernährung aus ökonomischen Gründen gerechtfertigt. Die höheren Ernährungskosten der Immunonutrition werden durch die geringeren Behandlungskosten bei den Komplikationen wettgemacht, sodass ein zusätzlicher Erlös von 332 € pro Patient mit Komplikation(en) unter Nutzen der Immunonutrition errechnet wurde.

Abstract

Background: Aim of this study was to evaluate the economic impact of immunonutrition given perioperatively in patients undergoing major gastrointestinal surgery under G-DRG conditions. Material and methods: Basis of this evaluation were clinical and economical data from two prospective, randomised, double-blind, placebo controlled multi centre studies published by Senkal et al. in the years 1995 and 1998. Direct resource use for diagnosis and therapy of the postoperative complications during hospital stay were assessed and valued with actual (year 2002) hospital purchasing prices or by using the DKG-NT tariff. Indirect costs (e. g. for work absenteeism) were not included. The complication costs were compared with the actual G-DRG based reimbursement rates and differences between immune and control nutrition were evaluated. Results: 69 of 308 patients developed postoperative complications, 27 out of 155 patients in the immunonutrition group and in 42 out of 153 patients in the control group (p = 0.012). The average cost including nutrition per patient treated for treating complications were 563 vs. 979 Euro (€) (immunonutrition vs. control). A considerably better cost effectiveness of 681 € was calculated for the immunonutrition group compared to 1350 € in the control group. The total complication related costs per group amounted to 67,438 € in the immunonutrition group vs. 147,076 € in the control group, with nutrition included 87,212 vs. 149,837 €. Complication reimbursement based on G-DRG was calculated to be 178,084 € in the immunonutrition group and 277,259 € in the control group. For each patient with postoperative complication(s) a reimbursement surplus of 3366 € remained in the immunonutrition group and of 3034 € in the control group. Conclusion: Based on the present economic evaluation, the use of immunonutrition can be recommended also under G-DRG conditions. Even with higher nutritional costs the perioperative use of immunonutrition results in an additional surplus of 332 € per patient with postoperative complication(s) compared to control.
