Zusammenfassung
Bericht über Jod-125-Plaque zur Behandlung von 9 Aderhautmelanomen mit Prominenz von 5-8,7 mm. Tumorlokalisation 2 × in Makulanähe, 2 × in Makula reichend, 1 × vom Papillenrand 1 mm entfernt. Bestrahlungsdosis pro Stunde 0,3-0,6 Gy, Tumorapex 70 Gy, Tumorbasis 370 Gy, Plaque-Liegezeit 1 - knapp 2 Wochen. Nachbeobachtungszeit 1-1Ÿ Jahre. Postoperativ Regression aller 9 Melanome; Abnahme der Prominenz um 51% nach 1 Jahr. Nachfolgend keine Bulbusverkleinerung, Katarakt, Rubeosis iridis oder Opticusatrophie, jedoch 2 × Visusabfall : 1 × vorübergehend (Visusanstieg nach Laserkoagulation von Neovaskularisation), 1 × bleibend (diskrete subretinale Gliose in restlicher nicht von Bestrahlungsnarbe erfaßter Makulahälfte). Visus 6 Monate postoperativ entspricht dem nach 1 bzw. 1Ÿ Jahren und beträgt bei 5 der 9 Patienten 0,2-0,6. Es wird versucht, anatomische und funktionelle Ergebnisse mit „Tumor-Chirurgie in kleinen Schritten”, realisiert durch Jod-125-Plaque-Behandlung, in Zusammenhang zu bringen.
Summary
The present paper reports on the treatment of 9 choroidal melanomas with a prominence of 5 to 8.7 mm, using iodine-125 plaque. Follow-up: 1-1Ÿ years. Radiation dose per hour: 0.3-0.6 Gy, tumor apex 70 Gy, tumor basis 370 Gy; plaque in situ 1 to 2 weeks. Localization of tumor: 2 × close to macula, 2 × extending into macula, 1 × 1 mm from disc. Postoperative regression of all 9 melanomas, by 51% after 1 year. Postoperatively no reduction in bulbus size, rubeosis iridis, cataract, or optic atrophy; however, 2 × reduced visual acuity: 1 × temporary (increase after laser coagulation of neovascularization), 1 × permanent (discrete subretinal gliosis in residual macular half not extending into radiation scar). Visual acuity 6 months postoperatively equal to that after 1-1Ÿ years and in 5 out of the 9 patients 20/100 to 20/30. An attempt is made to correlate the anatomical and functional results with the “tumor surgery in small steps” achieved with iodine-125 plaquing.
