Zusammenfassung

Einleitung

Die Corona-Pandemie hat die bestehenden Schwächen im Öffentlichen Gesundheitsdienst (ÖGD) offengelegt und verdeutlicht, dass der ÖGD fachlich moderner werden muss. Hierzu ist zukünftig ein wissenschaftliches Arbeiten im ÖGD entscheidend. Ziel dieses Positionspapiers ist es, eine klare Definition, die Aufgaben und die erforderlichen Voraussetzungen für den Aufbau und die Weiterentwicklung eines kommunalen Lehr- und Forschungsgesundheitsamts (LFGA) zu formulieren.

Methode

Am 11.12.2024 hat die Region Hannover hierzu eine hybride interkommunale Konferenz ausgerichtet. In Fokusgruppen wurden Zielfragen bearbeitet. Auf der Basis der Ergebnisse wurde ein Positionspapier erstellt, das im schriftlichen Umlauf konsentiert und am 20.02.2025 von der Autor/innen-Gruppe angenommen wurde.

Ergebnisse

Ein (kommunales) LFGA ist definiert als ein Gesundheitsamt, das eine innovative Schnittstelle zwischen Praxis, Lehre an Hochschulen und forschender Wissenschaft darstellt. Es hat die Aufgabe, die praktische Tätigkeit im Gesundheitsamt mit Hochschullehre und Forschung zu verbinden. So bietet es beispielsweise für den Studiengang „Humanmedizin“ Famulaturen und Wahlabschnitte des Praktischen Jahres sowie Kompetenzen für die fachärztliche Weiterbildung. Die Qualifizierung und Weiterentwicklung der Mitarbeitenden ist ein weiteres zentrales Anliegen. Zur eigenen Qualitätssicherung arbeiten LFGA Kommunen-übergreifend zusammen. Ideale Voraussetzung für ein LFGA ist die vertragliche Kooperation mit einer medizinischen Fakultät und auch anderen Hochschulen bzw. Fakultäten, beispielsweise mit der Einrichtung von Brückenprofessuren oder der Anbindung des LFGA als „An-Institut“. Meist handelt es sich bei einem LFGA um ein großes Gesundheitsamt (z. B. ein Großstadtgesundheitsamt) oder ein Netzwerk von Gesundheitsämtern.

Schlussfolgerung

Die Entwicklung zu einem LFGA stärkt das Gesundheitsamt vor Ort, in dem lokale Lösungen entwickelt werden und der Vielfalt der regionalen Herausforderungen auch mit Vielfalt in den wissenschaftlichen Fragestellungen begegnet werden kann. Der ÖGD erhält perspektivisch Innovationskraft und im Interesse der regionalen Anwendungsorientierung eine deutlich stärkere Kompetenz in der wissenschaftlichen Forschung und ärztlichen Weiterbildung, die für das Wohl der Bevölkerung positiv ist.

Abstract

Introduction

The corona pandemic has exposed numerous existing weaknesses in the Public Health Service (PHS). This clearly pointed out that the PHS requires modernization. In the future scientific methods will become even more crucial within the PHS. The aim of this article, among other things, is to formulate a clear definition, of a municipal teaching and research health department (TRHD). We also desire to precisely define the scope of the tasks and the necessary requirements for the establishment and further development of a TRHD.

Method

On December 11, 2024, the Hanover Region hosted an inter-municipal conference including participants who also attended the conference remotely. Within the focus groups questions regarding our goals were adressed. Based on these results, an article was formulated and circulated among the participants. The authors agreed upon and accepted the final version of this paper on February 20, 2025.

Results

A municipal TRHD is defined as a health department that represents an innovative interface between practical efforts on the one hand and university-level teaching and research on the other hand. That means, its task is to combine the two. For example, it offers internships and electives during the last year of medical school. The qualification and further development of employees is another central concern. TRHD municipalities work together to ensure and control their own quality. The ideal prerequisite for a TRHD is contractual cooperation with an university medical faculty and various other faculties. Valuable here would be the establishment of „bridging professorships“ and connecting the TRHD with the university as an affiliated institute. Ultimately, a TRHD requires a solid structural foundation in order to establish itself in the scientific field. This also includes connecting with a regionally important health department or a network of health departments.

Conclusion

The development of a TRHD strengthens every local health department by developing local solutions and addressing the diversity of regional challenges. This will also increase diversity in the applied scientific approach. With a regionaly-oriented implementation the future of a TRHD will gain innovative strength and significantly greater self-sufficiency in scientific research. This will also prove to be indispensable for the wellbeing of our population in the future.
