Wissenstransfer zu patientenorientierter Versorgung durch den Aufbau eines internationalen Netzwerks

Zusammenfassung

Hintergrund

Die patientenzentrierte Versorgung (PCC) ist ein Grundprinzip einer hochwertigen Gesundheitsversorgung. In letzter Zeit hat die PCC in vielen Ländern zunehmend an Bedeutung gewonnen. Im Jahr 2006 erklärte Chile die PCC zu einer tragenden Säule seines Gesundheitssystems. Die Umsetzung der PCC im chilenischen Gesundheitswesen hinkt jedoch noch hinterher. Dazu tragen unter anderem folgende Faktoren bei: 1) die Komplexität des Konzepts, 2) die gegenseitige Abhängigkeit zwischen Gesundheitsaktivitäten und politischen Vorschriften und 3) die Notwendigkeit eines kulturellen Wandels in der Gesundheitspolitik und -versorgung. In diesem Beitrag werden Erfahrungen vorgestellt, wie wir versucht haben, einige dieser Herausforderungen durch den Aufbau eines internationalen Netzwerks für PCC anzugehen.

Methoden

Im Rahmen eines Kooperationsprojekts zwischen Forschern und Klinikern aus Chile und Deutschland haben wir das Internationale Netzwerk für patientenzentrierte Versorgung (PCC NET) gegründet. Das Netzwerk bot Wissensaustausch und Schulungen auf verschiedenen Wegen an.

Ergebnisse

Der Wissensaustausch wurde durch die Entwicklung und den Start einer Website als Plattform für den Wissensaustausch gefördert. Darüber hinaus wurde eine Reihe von Online-Seminaren zum Thema PCC organisiert, die jeweils von einem internationalen Experten auf diesem Gebiet geleitet wurden. Im Januar 2023 wurden mehrere Präsenzworkshops in Chile entwickelt und durchgeführt. Insgesamt nahmen 51 Personen an den Workshops teil. Dabei handelte es sich entweder um Gesundheitsfachkräfte, die für die Umsetzung von PCC im Land verantwortlich sind und für das Gesundheitsministerium oder in der Primärversorgung in Chile arbeiten. Wir haben die Workshops anhand von Umfragen unter den Teilnehmern evaluiert, die die Workshops positiv bewerteten. Im Januar 2024 fand in Santiago de Chile die erste lateinamerikanische Konferenz zum Thema patientenzentrierte Versorgung statt. An den beiden Konferenztagen gab es vier Keynotes, zwei Rundtischgespräche, fünf Workshops, 31 Vorträge und 19 Posterpräsentationen aus Forschung und klinischer Praxis mit 196 Teilnehmern vor Ort.

Schlussfolgerung

Die Einrichtung des PCC NET hat die internationale Zusammenarbeit, den Wissensaustausch und den Kapazitätsaufbau für die Umsetzung der PCC in Chile erleichtert. Durch verschiedene Kommunikationsstrategien wie Workshops, Seminare und eine Konferenz konnten das Bewusstsein und das praktische Wissen von Fachkräften im Gesundheitswesen und die Dynamik in Politik und Praxis aufrechtzuerhalten.
