Congenital Segmental Dilatation of the Duodenum - Differential Diagnosis for an Antenatally-Diagnosed Intra-Abdominal Cyst

Summary

We report the third case of segmental dilatation of the duodenum and the first to be diagnosed antenatally. The lesion presented as an intra-abdominal cyst on an antenatal scan at 17 weeks and was followed up with more specific investigations after birth. The final diagnosis was obtained only at laparotomy where the dilated segment was resected. The patient is well at eighteen month follow-up. Segmental dilatation of the gastrointestinal tract, although rare, should be considered in the differential diagnosis of antenatally discovered intra-abdominal cysts.

Résumé

Nous faisons notre rapport sur le troisième cas de dilatation segmentaire du duodénum et le premier qui a été diagnostiqué dans la période prénatale. La lésion s'est présentée comme un kyste intra-abdominal sur l'écho prénatale à dix-sept semaines. Le diagnostic définitif a été fait par une laparotomie et le segment dilaté a été reséqué. Dix-huit mois plus tard la patiente va bien. La dilatation segmentaire de l'appareil digestif, bien qu'elle soit rare, doit être prise en considération dans le diagnostic différentiel des kystes intra-abdominaux découverts dans la période prénatale.

Resumen

Presentamos el tercer caso de dilatación segmentaria congénita del duodeno y el primero diagnosticado prenatalmente. La lesión se presentó como un quiste intraabdominal en una ecografía a las 17 semanas de gestación y se investigó más específicamente después del nacieminto. El diagnóstico final se hizo tras resecar el segmento dilatado. El paciente está bien 18 meses más tarde. La dilatación segmentaria congénita del intestino, aunque rara debe sere considerada en el diagnóstico diferencial de los quistes intra-abdominales diagnosticados prenatalmente

Zusammenfassung

Es wird der dritte Fall einer segmentalen Dilatation des Duodenums vorgestellt, der zudem erstmalig bereits pränatal diagnostiziert werden konnte. Während der Schwangerschaft wurde eine intraabdominelle Zyste im Alter von 17 Wochen diagnostiziert, die endgültig im Rahmen der postpartalen Laparotomie als segmentale Dilatation des Duodenums identifiziert werden konnte. Es wurde eine sparsame Resektion unter Schonung der Papille durchgeführt. Postoperativ ist der Patient 18 Monate nach dem Eingriff unauffällig. Segmentale Dilatationen des Magen-Darm-Traktes sind ungewöhnlich selten, insbesondere auch im Bereich des Duodenums. Sie sollten jedoch bei der Differentialdiagnose intraabdomineller Zysten mit berücksichtigt werden.
