The Effect of ATP-MgCl2 on Prevention of Reperfusion Injury After Unilateral Testicular Torsion

Summary

This study was designed to investigate the effect of ATP-MgCl2 administered before and after detorsion on the prevention of reperfusion injury after unilateral testicular torsion. The rats were divided into six groups, each containing six rats. Torsion was created by rotating the left testes 720° in a clockwise direction. Group 1 functioned as a control group. Torsion only was carried out in Group 2. Detorsion was carried out in Group 3. ATP-MgCl2 (100 µmol/kg) was injected intravenously immediately before detorsion in Group 4. ATP-MgCl2 (100 µmol/kg) was injected intravenously immediately after detorsion in Group 5. Saline was injected intravenously immediately after detorsion in Group 6. The effect of ATP-MgCl2 on reperfusion injury was investigated by determining the levels of thiobarbituric acid-reactive substances (TBAR) and resulting lipid peroxidation in the bilateral testicular tissue. Testicular torsion and detorsion caused a significant increase in the TBAR levels in the bilateral testicular tissue. TBAR levels decreased to approximately normal levels in Groups 4 and 5. It is concluded that if reperfusion injury has occurred in both testes after unilateral testicular torsion, ATP-MgCl2 administered before or after detorsion may prevent reperfusion injury in testicular torsion.

Résumé

Cette étude à été réalisée pour apprécier les effets de ATP-MgCl2 administrée avant et après torsion pour prévenir les lésions de reperfusion après torsion testiculaire unilatérale. Les rats étaient divisés en 6 groupes de 6 rats. La torsion a été crée par rotation du testicule gauche de 720° dans le sens des aiguilles d'une montre. Le groupe A était le groupe contrôle. Une torsion seulement a été réalisée dans le groupe 2. La détorsion était réalisée dans le groupe 3. ATP-MgCl2 (100 µmol/kg) était injectée par voie intraveineuse immédiatement avant la détorsion dans le groupe 4.

ATP-MgCl2 (100 µmol/kg) était injectée par voie intraveineuse immédiatement après la détorsion dans le groupe 5. Le sérum physiologique était injecté par voie intraveineuse immédiatement après détorsion dans le groupe 6.

L'effet de l'ATP-MgCl2 sur les lésions de reperfusion étaient apprécié en déterminant le taux de substances réactives d'acide thiobarbiturique (TBAR) résultant de la peroxydation lipidique dans le tissu testiculaire. La torsion et la détorsion du testicule entraîne une augmentation significative des taux de TBAR dans le tissu testiculaire. Les taux de TBAR sont diminués jusqu'à un taux presque normal dans les groupes 4 et 5.

Nous concluons que les lésions de reperfusion surviennent dans les deux testicules après torsion testiculaire unilatérale, et que l'ATP-MgCl2, qui est administrée avant ou après la détorsion peut prévenir les lésions de reperfusion.

Resumen

Estudio diseñado para investigar el efecto del ATP-MgCl2 administrado antes y después de la detorsión en la prevención del daño por reperfusión tras torsión testicular unilateral.

Se dividieron las ratas en 6 grupos de 6 cada uno. Se creó la torsión rotando el testículo izquierdo 720º en sentido horario. El grupo 1 fue un sham. En el grupo 2 solamente se torsionó el testículo. En el grupo 3 se detorsionó. En el grupo 4 se inyectó ATP-MgCl2 (100 µmol/kg) inmediatamente antes de la detorsión. En el grupo 5 se inyectó la misma cantidad inmediatamente después de la detorsión. En el grupo 6 se inyectó salino inmediatamente después de la detorsión. Se investigó en ambos testículos el efecto del ATP-MgCl2 sobre el daño de reperfusión determinando los niveles de substancias reactivas al ácido tiobarbitúrico (TBAR) resultantes de la peroxidación lipídica.

La torsión testicular y la detorsión causaron aumentos significativos de los niveles de TBAR en el tejido testicular de ambos lados. Los niveles de TBAR decrecieron aproximadamente hasta niveles normales en los grupos 4 y 5.

Concluímos que el daño por reperfusión ocurrió en ambos testículos tras la torsión testicular unilateral y que el ATP-MgCl2 administrado antes y después de la detorsión puede evitar el daño por reperfusión en la torsión testicular.

Zusammenfassung

Die vorliegende Untersuchung wurde durchgeführt, um den Effekt von ATP-MgCl2 vor und nach der Detorsion einer unilateralen Hodentorsion, insbesondere im Hinblick auf Reperfusionsschäden zu überprüfen. Hierzu wurden 6 verschiedene Gruppen von Ratten gebildet. Die Hodentorsion wurde durch eine Verdrehung der Samenstranggebilde um 720 Grad im Uhrzeigersinn erzielt. Gruppe 1 war die Kontrollgruppe. In Gruppe 2 wurde lediglich eine Hodentorsion durchgeführt, in Gruppe 3 zusätzlich eine Detorsion. Gruppe 4 erhielt 100 µmol/kg Körpergewicht ATP-MgCl2 intravenös unmittelbar vor der Detorsion. Gruppe 5 hingegen erhielt dasselbe Medikament nach der Detorsion. Bei Gruppe 6 wurde lediglich physiologische Kochsalzlösung injiziert. Die Wirkung von ATP-MgCl2 auf einen möglichen Reperfusionsschaden wurde durch Bestimmung der Spiegel Thiobarbitursäure-reaktiver Substanzen bestimmt, die eine Lipidperoxidation im Hodengewebe verursachen. Ergebnisse: Die Hodentorsion und Detorsion verursachten einen signifikanten Anstieg der Thiobarbitursäure-reaktiven Substanzen im beidseitigen Hodengewebe. In den Gruppen 4 und 5 hingegen, bei denen ATP-MgCl2 verabfolgt wurde, waren diese Spiegel signifikant niedriger oder normal. Schlussfolgerung: Hieraus ergibt sich, dass die Gabe von ATP-MgCl2 vor und nach der Detorsion einer Hodentorsion den Reperfusionsschaden bei dieser Erkrankung verringern oder ganz verhindern kann.
