Laparoscopic Treatment of Varicoceles in Children

Summary

Purpose: The aim of this study was to assess the results of the laparoscopic treatment of varicoceles. Material and Methods: 92 observations were received, however 5 patients underwent a simultaneous associated open surgery; 90 varicoceles were evaluated (3 bilateral varicoceles). The dilated spermatic veins were treated by clips and partial resection in 74.7 %, by coagulation and partial resection in 21 %, and by clips in 4.3 %. The spermatic artery was ligated in 60 varicoceles and preserved in 28; in 2 cases it was not specified. The results were studied with a mean follow-up of 11.1 months (2 to 36 months). Results: The varicocele disappeared in 76 cases (84.4 %) and diminished significantly in 12 cases (13.2 %) where it was large before treatment. Two middle-sized varicoceles were not modified by the treatment. Therefore, the results must be considered good or excellent in 97.6 % of cases. The spermatic artery was preserved in 28 cases and the size of the varicocele was not good in 9 (32.1 %); it was ligated in 60 cases and the size of the varicocele was not good in 5 cases (8.4 %) - p < 0.01. In 42 cases the testicular size was noted before and after treatment, and likewise the conservation or not of the spermatic artery; the 30 artery ligations produced no testicular atrophy; only 3 hypotrophic testes were noted with a loss of size of between 8.4 and 59 % with regard to the controlateral testis. In 27 cases, the testicular size remained normal or increased if it was hypotrophic before treatment; there was a good result in 90 % of cases with no atrophy. When the spermatic artery was preserved (12 cases), 1 hypotrophy occurred with a loss of size of 16.7 % - NS. Conclusion: The laparoscopic treatment of varicoceles is an efficient technique and the complete ligation of the whole vascular spermatic pedicle above the vas deferens offers excellent success, without atrophy.

Résumé

But: Apprécier de façon prospective les résultats du traitement laparoscopique des varicocèles. Matériel et Méthodes: 87 observations ont été retenues parmi les 92 reçues, 5 ont eu un traitement laparoscopique associé à un geste à ciel ouvert, inguinal ou scrotal; dans 3 cas, il s'agissait de formes bilatérales et ce sont donc 90 varicocèles qui ont été jugées. Les veines spermatiques dilatées ont eu clips + résection partielle dans 74,7 % des cas, coagulation + résection partielle dans 21 % et clips seuls dans 4,3 %. L'artère spermatique a été sectionnée dans le même temps 60 fois et conservée 28 fois; 2 fois cela n'a pas été précisé. Les résultats ont été appréciés avec un recul moyen de 11,1 mois (2 à 36 mois). Résultats: La varicocèle a disparu dans 76 cas (84,4 %) et a nettement diminué de taille dans 12 cas (13,2 %) alors qu'elle était volumineuse avant traitement. Deux varicocèles de taille moyenne n'ont pas été modifiées par le traitement. Les résultats ont donc été bons ou excellents dans 97,6 % des cas. L'artère spermatique a été conservée 28 fois et le résultat n'a pas été excellent 9 fois (32,1 %); elle a ¿eté sectionnée 60 fois et le résultat n'a pas été excellent 5 fois (8,4 %) - p < 0,01. Dans 42 observations, la taille du testicule sousjacent à la varicocèle a été notée avant et après laparoscopie selon que l'artère spermatique ait été conservée ou non. La section de l'artère n'a eu aucune conséquence 27 fois sur 30 (90 %) et il a été constaté une hypotrophie dans 3 cas, avec perte de 8,4 à 59 % du volume par rapport au testicule controlatéral; aucune atrophie n'a été relevée; dans 11 cas où le testicule était hypotrophique au départ et où l'artère avait été sectionnée, il y a même eu récupération, l'hypotrophie ayant disparu 5 fois et étant améliorée 6 fois. Pour les 12 autres varicocèles où l'artère a été conservée, une seule hypotrophie a été notée, avec perte de 16,7 % du volume (NS). Conclusion: Le traitement laparoscopique des varicocèles est une excellente alternative et la section de l'artère spermatique au cours du traitement améliore les résultats, sans conséquences fâcheuses pour le testicule sous-jacent.

Resumen

Objetivo: El objetivo de este estudio fue evaluar los resultados del tratamiento laparoscópico de los varicoceles. Material y Métodos: Revisamos 92 historias de las que 5 correspondían a pacientes que habían sufrido simultáneamente cirugía abierta. Evaluamos 90 varicoceles (3 bilaterales). Las venas espermáticas dilatadas se trataron con clips y resección parcial en el 74,7 %, por coagulación y resección parcial en el 21 % y por clips en el 4,3 %. La arteria espermática se ligó en 60 varicoceles y se preservó en 28. En 2 casos no se especificó el tipo de operación. Los resultados fueron estudiados con un seguimiento medio de 11,1 meses (2 a 36). Resultados: El varicocele desapareció en 76 casos (84.4 %) y disminuyó significativamente en 12 (13,2 %) en los que era muy grande antes del tratamiento. Los varicoceles de tamaño mediano fueron modificados por la operación. Por tanto los resultados deben ser considerados buenos o excelentes en el 97,6 % de los casos. La arteria espermática se preservó en 28 niños con resultado malo en 9 (32,1 %). Se ligó en 60 casos con resultado malo en 5 casos (8,4 %) E > 0.01. En 42 casos se midió el tamaño testicular antes y después del tratamiento. Las 30 ligaduras arteriales no produjeron ninguna atrofia testicular. Solo fueron vistos 3 testículos hipotróficos con una pérdida de tamaño de entre 8,4 y 59 % con respecto al testículo contralateral. En 27 casos el tamaño testicular permaneció normal o aumentado si era hipotrófico antes del tratamiento. Hubo buen resultado en 90 % de los casos sin atrofia. Cuando la arteria espermática se preservó (12 casos) ocurrió una hipotrofia con pérdida de tamaño (16,7 %, n.s.). Conclusión: El tratamiento laparoscópico del varicocele es una técnica eficaz y la ligadura completa del pedículo vascular por encima del deferente ofrece excelentes resultados sin atrofia.

Zusammenfassung

Ziel der Studie: Beurteilung der Ergebnisse bei laparoskopischer Therapie von Varikozelen. Methoden und Material: 87 von 92 Observationen wurden in diese Studie eingeschlossen. 5 hatten einen kombinierten laparoskopischen und konventionellen, entweder skrotalen oder inguinalen Zugang; es handelte sich um beidseitige Formen, es wurden also 90 Varikozelen beurteilt. Die dilatierten V. spermatica wurden geklippt und teilreseziert in 74,7 % der Fälle, koaguliert und teilreseziert in 21 % und nur geklippt in 4,3 %. In 60 Fällen kam es gleichzeitig zu einer Sektion der A. spermatica, in 28 Fällen blieb diese erhalten; in zwei Fällen gab es hierzu keine Präzisierungen. Die Ergebnisse wurden nach durchschnittlich 11,1 (2 - 36) Monaten beurteilt. Ergebnisse: Verschwinden der Varikozele in 76 Fällen (84,4 %) und deutliche Verkleinerung in 12 Fällen (13,2 %), bei vor der Behandlung sehr großem Volumen. Zwei mittelgroße Varikozelen wurden durch die Behandlung nicht verändert. Die Ergebnisse waren also gut oder exzellent in 97,6 % der Fälle. Die A. spermatica wurde 28-mal erhalten und das Ergebnis war 9-mal nicht exzellent (32,1 %); sie wurde 60-mal sektioniert und das Ergebnis war 5-mal nicht exzellent (8,4 %) - p < 0,01. In 42 Observationen wurde die Hodengröße bei bestehender Varikozele vor und nach der Laparoskopie und abhängig von der Sektion der A. spermatica ermittelt. Die Sektion der A. spermatica hatte 27-mal von 30 (90 %) keinerlei Konsequenz, und eine Hypotrophie wurde in 3 Fällen festgestellt, mit einem Verlust von 8,4 bis 59 % des Volumens im Vergleich zum kontralateralen Hoden; in keinem Fall kam es zu einer Atrophie; in 11 Fällen mit hypotrophem Hoden vor dem Eingriff und bei denen die A. spermatica sektioniert wurde, kam es sogar zu einer Verbesserung, die Hypotrophie verschwand in 5 Fällen, und 6-mal wurde sie verbessert. Bei den 12 anderen Varikozelen, bei denen die Arterie erhalten blieb, kam es nur zu einer Hypotrophie mit Verlust von 16,7 % des Hodenvolumens (NS). Zusammenfassung: Die laparoskopische Therapie der Varikozelen ist eine exzellente Alternative, und die Sektion der A. spermatica im Rahmen des Eingriffs verbessert die Ergebnisse, ohne unerfreuliche Konsequenzen für den betroffenen Hoden.
