Ultrasonographic Follow-Up of Infantile Hypertrophic Pyloric Stenosis after Pyloromyotomy: A Controlled Prospective Study

Abstract

Although the ultrasonographic assessment of hypertrophic pyloric stenosis (IHPS) has become the main radiological method for this disease, our knowledge about postoperative ultrasonographic follow-up is very limited. To evaluate the ultrasonographic outcome of the pylorus, we performed ultrasonographic measurements of the pylorus in 22 children with IHPS, before and after the operation in a prospective trial.

The sonograms following surgery were undertaken at the end of the 1st postoperative week and of the 1st, 3rd and 6th month postoperatively. Three main ultrasonographic parameters were used: pyloric muscle thickness (MT), pyloric diameter (PD) and pyloric length (PL).

The values of the pylorus, especially the “MT” measurements, began to decrease almost by the end of the first postoperative week, and were normal by the 3rd month in the majority of cases. However, the averages of the PL and PD values never returned to normal throughout the 6 months follow-up. There was a highly significant difference between the average values of pyloric muscle thickness at admission and at the 6th month after surgery (p < 0.001).

Within one month after surgery, the ultrasonographic parameters returned to the levels of those in control infants. However, except for MT, they did not return to their normal ranges. Therefore, MT is the most useful parameter for ultrasonographic postoperative evaluation if the normal ultrasonographic values, as given in the literature, are used.

Résumé

Quoique le bilan échographique soit devenu la principale méthode de diagnostic de la sténose hypertrophique du pylore (SHP), notre connaissance au sujet du devenir post-opératoire est limité. Pour évaluer le devenir ultrasonographique du pylore, nous avons réalisé des mesures ultrasonographiques du pylore chez 22 enfants avec SHP avant et après l'opération. L'échographie suivant la chirurgie était réalisée à la fin de la 1 ère semaine, au 1er, au 3 ème et au 6 ème mois post-opératoire. Trois principaux paramètres ultrasonographiques étaient utilisés: l'épaisseur du muscle pylorique (MT), le diamètre du pylore (PD) et la longueur du pylore (PL).

Les valeurs du pylore, et particulièrement le MP commencent à diminuer déjà à la fin de la 1 ère semaine post-opératoire et deviennent normales au 3 ème mois dans la majorité des cas. Toutefois, les moyennes de PL et PD ne reviennent jamais à la normale pendant les 6 premiers mois post-opératoires. Il y avait une différence significative entre les valeurs moyennes de l'épaisseur du muscle pylorique à l'admission et au 6 ème mois post-opératoire (p < 0.001).

Pendant le 1er mois après la chirurgie, les paramètres ultrasonographiques reviennent à des niveaux observés dans le groupe contrôle. Toutefois, excepté MT, ils ne manquent pas de revenir à la normale. Ainsi, MT est le plus utile des paramètres pour l'évaluation post-opératoire échographique.

Resumen

Aunque la evaluación ecográfica de la estenosis hipertrófica del píloro (IHPS) se ha convertido en el método de imagen más usado en esta enfermedad nuestro conocimiento sobre la evolución postoperatoria es muy limitado. Con este fin hemos tomado medidas ultrasonográficas del píloro en 22 niños con IHPS antes y después de la operación en un estudio prospectivo. Las ecografías tras la cirugía fueron realizadas al final de la primera semana y en el primero, tercero y sexto mes postoperatorios. Se usaron tres parámetros ecográficos: el espesor de la pared muscular (MT), el diámetro pilórico (PD) y la longitud pilórica (PL).

Los valores hallados, especialmente el MT comenzaron a disminuir casi al final de la primera semana postoperatoria y se hicieron normales en el tercer mes en la mayoría de los casos. Sin embargo, los valores medios del PL y PD nunca volvieron a la normalidad en los 6 meses de seguimiento estudiados. Hubo una diferencia muy altamente significativa entre los valores medios del espesor del músculo pilórico al ingreso y en el sexto mes tras la cirugía (p < 0.001).

Dentro del primer mes postoperatorio los parámetros ecográficos volvieron a los niveles de sujetos control. Sin embargo, salvo MT, no volvieron a rangos normales. En consecuencia MT es el parámetro más útil para la evaluación ultrasonográfica postoperatoria.

Zusammenfassung

Obwohl die Ultraschallsonographie die Diagnostik der Wahl bei der hypertrophen Pylorusstenose darstellt, gibt es keine postoperativen Ultraschallkontrollen. Die Frage, wie sich der Pylorus postoperativ verändert und in welchem Zeitraum normalisiert, ist daher offen.

Methodik

In der vorliegenden Arbeit wurde bei 22 Kindern mit hypertropher Pylorusstenose prä- und postoperativ die Konfiguration des Pylorus überprüft. Die Ultraschalluntersuchungen erfolgten 1 Woche nach dem Eingriff sowie 3 und 6 Monate später. Drei ultrasonographische Parameter wurden überprüft: Die Dicke des Pylorusmuskels (MT), der Durchmesser des Pylorus (PD) und die Länge des Pyloruskanals (PL).

Ergebnisse

Diese drei Parameter, insbesondere die Muskeldicke, begannen 1 Woche nach dem operativen Eingriff abzunehmen und normalisierten sich in der Mehrzahl der Fälle 3 Monate postoperativ. Allerdings kehrten die Durchschnittswerte des Pylorusdurchmessers und der Länge des Pyloruskanals auch nicht in einem Beobachtungszeitraum von 6 Monaten zur Norm zurück. Der Unterschied hinsichtlich der Dicke des Pylorusmuskels bei Aufnahme 6 Monate später war signifikant (p < 0,001).

Schlussfolgerungen

Ein Monat nach dem Eingriff kehrten somit die Ultraschallparameter auf die Größenordnung gesunder Kinder zurück. Die Ausnahme war jedoch die Pylorusmuskeldicke (MT), die nicht auf die Normwerte zurückging. Damit muss die Pylorusmuskeldicke als der nützlichste Ultraschallparameter für eine postoperative Kontrolle angesehen werden.
