Colonic Duplication: Diagnostic Dilemma

Abstract

Colonic duplication is a rare congenital anomaly which presents as a diagnostic and therapeutic challenge. We report a 5-year-old boy with colonic duplication who required multiple hospital admissions, multiple diagnostic tests, and evaluation by various clinical specialists before the diagnosis was made intraoperatively. He was known to have left renal hypoplasia, low spinal abnormalities, hypoplasia of the left hemipelvis, and mild hypoplasia of the left lower limb, all of which were considered to be a variant of caudal regression syndrome.

Résumé

La duplication colique est une anomalie congénitale rare qu'il est difficile de diagnostiquer et à traiter. Nous rapportons le cas d'un garçon de 5 ans avec une duplication colique qui a nécessité plusieurs hospitalisations, plusieurs examens et une évaluation par plusieurs spécialistes avant que le diagnostic n'ait été fait en per-opératoire. Il avait une hypoplasie du rein gauche, une anomalie de la partie de la colonne vertébrale, une hypoplasie de l'hémi-pelvis gauche, une hypoplasie du membre inférieur gauche, tout ceci était considéré comme une variation d'un syndrome de régression caudale.

Resumen

La duplicación del colon es una malformación rara que constituye un reto diagnóstico y terapeútico. Presentamos el caso de un niño de 5 años con duplicación del colon que requirió varias hospitalizaciones, pruebas diagnóstico y evaluación por especialístas antes de llegar al diagnóstico durante la operación. El paciente tenía hipoplasia renal izquierda, malformación raquídea inferior, hipoplasia de la pelvis izquierda así como leve hipoplasia del miembro inferior izquierdo por lo que se le consideró como una variante del síndrome de regresión caudal.

Zusammenfassung

Die Kolonduplikatur ist eine seltene angeborene Anomalie, die eine diagnostische und therapeutische Herausforderung darstellt. Wir berichten über einen 5 Jahre alten Jungen mit Kolonduplikatur, der zahlreiche Krankenhausaufenthalte, diagnostische Tests und Untersuchungen durch verschiedene klinische Spezialisten erfahren hat, bevor die Diagnose intraoperativ gestellt wurde. Bei ihm waren eine Nierenhypoplasie links, eine Deformität der unteren Wirbelsäule, eine Hypoplasie der linken Hüfte sowie eine leichte Hypoplasie der linken unteren Extremität bekannt, die allesamt für eine Variante eines kaudalen Regressionssyndroms gehalten wurden.
