Sigmoid Colon Vaginoplasty in Children

Abstract

Background: Vaginal construction is necessary for the patients with aplasia of Mullerian ducts, testicular feminisation and androgen insensitivity syndromes. Many methods of vaginal construction have been described. We report here the outcomes of six adolescent patients who underwent sigmoid colon vaginoplasty with special emphasis on the surgical technique and outcomes. Patients and Methods: Between 1990 and 2003, six patients underwent sigmoid vaginoplasty after a diagnosis of 5α-reductase deficiency (n = 3), testicular feminisation (n = 2) or vaginal atresia (n = 1). The mean age was 16 years (13 to 18). Wide spectrum antibiotics and whole-gut preparation were used in all cases. A 15 - 20 cm segment of sigmoid colon was pulled through the retrovesical tunnel. The proximal end was closed in two layers in patients with 5α-reductase deficiency and with testicular feminisation. A distal anastomosis was carried out to the opening made on the vaginal plate (5α-reductase deficiency) or on the tip of the shallow rudimentary vagina (testicular feminisation). The sigmoid segment was interposed between the blind end of the atretic vagina and the perineum in the patient with vaginal atresia. Patients were instructed to perform daily vaginal irrigation. The neovagina was examined and calibrated under anaesthesia. No routine vaginal dilatation was recommended. Results: All but one patient had an uneventful postoperative period and were discharged within 7 - 8 days. All patients had an excellent cosmetic result with an appropriate vaginal length. One of the patients experienced late stenosis of the introitus which responded to dilatations. Mucus discharge was not a significant problem. The patient with vaginal atresia (Bardet-Biedl syndrome) experienced deep vein thrombosis, renal failure and sepsis, resulting in death. Conclusion: Sigmoid colon vaginoplasty is a special procedure which appears appropriate for the construction of a new vagina in children. A sigmoid colon neovagina meets all necessary criteria after a vaginoplasty. It provides an adequate diameter and length, and produces less scar tissue in the perineum. It is self-moistening, easily adaptable to the uterus, cervix and rudimentary atretic vaginal segments and does not require routine dilatation. Mild stenosis of the introitus can be treated by dilatations and revision can be easily performed in severely stenotic cases. On the other hand, the patient may face morbidity after laparotomy and other serious complications may occur due to accompanying diseases.

Etat de la Question: La vaginoplastie est indiquée chez les patientes porteuses d'une aplasie des canaux de Müller, d'un syndrome d'insensibilité aux androgènes ou d'autres pseudohermaphrodismes masculins. Plusieurs techniques de reconstruction vaginale ont été décrites. Nous rapportons ici la technique et les résultats de la vaginoplastie utilisant le colon sigmoïde chez six adolescentes. Patients et Méthodes: Entre 1990 et 2003, six patientes ont eu une vaginoplastie sigmoïdienne pour un déficit en 5α-réductase (n = 3), une insensibilité aux androgènes (n = 2) ou une atrésie vaginale (n = 1). L'âge moyen était de 16 ans (13 à 18 ans). Une antibiothérapie à large spectre et une préparation intestinale ont été réalisés dans tous les cas. Un segment de colon sigmoïde de 15 - 20 cm a été positionné dans la filière rétro-vésicale. L'extrémité proximale a été fermée en deux plans chez les patientes avec un déficit en 5α-réductase ou un testicule féminisant. Une anastomose distale a été réalisée avec la cupule vaginale (déficit en 5α-réductase) ou avec le cul-de-sac vaginal rudimentaire (testicule féminisant). Le segment sigmoïdien a été interposé entre le cul-de-sac terminal du vagin atrétique et le périné chez la patiente porteuse d'une atrésie vaginale. Les patientes étaient informées de la nécessité de réaliser une irrigation vaginale quotidienne. Le néo-vagin était examiné et calibré sous anesthésie générale. Une dilatation vaginale régulière n'était pas recommandée. Résultats: Toutes les patientes sauf une ont eu des suites opératoires simples et ont pu quitter l'hôpital à J7-J8 post-opératoire. Toutes les patientes ont eu un résultat cosmétique excellent, avec une longueur vaginale satisfaisante. Une des patientes a présenté une sténose de l'orifice vaginal qui a bien répondu aux dilatations. La patiente avec une atrésie vaginale (syndrome de Bardet-Bidl) est décédée de complications à type de thrombose veineuse profonde, insuffisance rénale et sepsis. Conclusion: La vaginoplastie colique sigmoïdienne est une technique spécifique qui semble appropriée pour la reconstruction vaginale chez l'enfant. Elle répond à tous les impératifs demandés à la création d'un néo-vagin. Elle donne un diamètre et une longueur adaptés tout en induisant le minimum de lésions des tissus périnéaux. Elle est spontanément humidifiée, facilement adaptable à l'utérus, le col et les segments vaginaux atrétiques rudimentaires et ne nécessite pas une dilatation régulière. Les sténoses modérées de l'orifice vaginal peuvent être traitées par des dilatations et des retouches chirurgicales peuvent être réalisées facilement dans les sténoses sévères. Il existe néanmoins une morbidité liée à la laparotomie, et des complications sévères peuvent survenir, liées à la pathologie initiale.

Resumen

Antecedentes: La reconstrucción vaginal es necesaria en pacientes con aplasia de conductos mullerianos, feminización testicular y síndrome de insensibilidad a los andrógenos. Se han descrito muchos métodos de reconstrucción. Presentamos el resultado en 6 pacientes adolescentes en quienes se reconstruyó la vagina con sigma destacando los aspectos técnicos y los resultados. Pacientes y Métodos: Entre 1990 y 2003 realizamos vaginoplastia con sigma en 6 pacientes diagnosticadas de déficit de 5 α-reductasa (n = 3), feminización testicular (n = 2) o atresia vaginal (n = 1). La edad media fue de 16 años (13 a 18). Se usaron antibióticos de amplio espectro y preparación intestinal total. Se descendió un segmento de sigma de 15 a 20 cm. a través de un túnel retrovesical. El extremo proximal se cerró en dos capas en los pacientes con déficit de 5 α-reductasa y feminización testicular. Se realizó una anastomosis distal a la apertura en el introito vaginal (déficit de 5 α-reductasa) o en el extremo de la vagina rudimentaria (feminización testicular). El segmento sigmoideo se interpuso entre el extremo ciego de la vagina atrésica y el periné en la paciente con atresia vaginal. Las pacientes fueron instruidas para realizar irrigación vaginal diaria. La neovagina se examinó y calibró bajo anestesia. No se hicieron dilataciones vaginales rutinarias. Resultados: Todas las pacientes menos una tuvieron un postoperatorio sin problemas y fueron dadas de alta entre el 7° y el 8° día. Todas tuvieron un resultado cosmético excelente con longitud vaginal apropiada. Una de las pacientes tuvo estenosis tardía del introito que respondió a dilataciones. La secreción mucosa no fue un problema significativo. La paciente con atresia vaginal (Síndrome de Bardet-Biedl) tuvo trombosis venosa profunda, fallo renal y sepsis que le llevó a la muerte. Conclusión: La vaginoplastia sigmoidea es un procedimiento apropiado para la construcción de una nueva vagina en niñas. La neovagina sigmoidea reune todos los criterios necesarios como plastia. Proporciona un calibre suficiente y una longitud adecuada y produce menos tejido cicatricial en el periné. Es húmeda, fácilmente adaptable al útero, al cervix o al segmentos atrésicos vaginales y no requiere dilatación rutinaria. Se puede tratar una estenosis menor del introito por dilataciones y se puede revisar fácilmente en los casos con estrechez grave. Por otra parte la paciente puede sufrir la morbilidad de la laparotomía y otras complicaciones serias que pueden acompañar a la enfermedad original.

Zusammenfassung

Bei Patienten mit Aplasie der Müllerschen Gänge, testikulärer Feminisierung und dem Syndrom der Androgenresistenz ist eine vaginale Rekonstruktion notwendig. Hierfür werden verschiedene Methoden beschrieben. In der vorliegenden Arbeit wird das Langzeitergebnis bei 6 inzwischen erwachsenen Patientinnen mit Sigmoidkolonvaginoplastik beschrieben. Patienten und Methodik: Die 6 Patienten wurden wegen folgender Diagnosen operiert: 5-α-Reduktasemangel (n = 3), testikuläre Feminisierung (n = 2) und Vaginalatresie (n = 1). Zum Operationszeitpunkt (1990 - 2003) waren die Mädchen durchschnittlich 16 Jahre alt (13 - 18 Jahre). In allen Fällen wurde eine Breitspektrumantibiotikabehandlung sowie eine vollständige Darmreinigung durchgeführt. 15 - 20 cm des Colon sigmoideum wurden durch einen retrovesikalen Tunnel durchgezogen. Das proximale Ende des Kolons wurde bei den Patienten mit 5-α-Reduktasemangel und testikulärer Feminisierung zweischichtig verschlossen. Das distale Ende wurde in die Vaginalplatte implantiert (5-α-Reduktasemangel) oder mit dem oberen Ende der rudimentären Vagina anastomisiert (testikuläre Feminisierung). Bei dem Mädchen mit Vaginalatresie wurde das Kolon zwischen dem blinden Ende der atretischen Vagina und dem Perineum implantiert. Alle Patienten wurden angehalten, täglich vaginale Spülungen vorzunehmen. Die Neovagina wurde bei der Nachuntersuchung unter Anästhesie kalibriert und untersucht. Routinemäßige Vaginalbougierungen wurden nicht empfohlen. Ergebnisse: Von den 6 Patientinnen zeigten 5 einen unauffälligen postoperativen Verlauf und konnten innerhalb von 7 - 8 Tagen entlassen werden. Alle Mädchen zeigten ein kosmetisch ausgezeichnetes Ergebnis mit einer ausreichenden Vaginallänge. Ein Mädchen entwickelte jedoch eine Spätstenose des Introitus und benötigte Dilatationen. Der Ausfluss von Schleim war kein wesentliches Problem für die Patientinnen. Das Kind mit Vaginalatresie (Bardet-Biedl-Syndrom) verstarb jedoch später an einer tiefen Venenthrombose mit Nierenversagen und Sepsis. Schlussfolgerung: Die Vaginoplastik mittels des Sigmoidkolons ist eine gute Methode, um die Vagina bei jugendlichen Mädchen zu rekonstruieren. Sie erfüllt alle Kriterien, die für eine Neovagina nach Vaginoplastik erforderlich sind. Das bedeutet, dass sie einen ausreichenden Durchmesser und Länge der Vagina garantiert und wenig Narbengewebe im Perineum produziert. Die Sigmoidvagina befeuchtet sich selbst, ist gut mit dem Uterus zu adaptieren, aber auch mit der Zervix und einem rudimentären Vaginalsegment. Routinemäßige Dilatationen sind nicht erforderlich. Eine leichte Stenosierung des Introitus kann durch Dilatationen oder operative Revision leicht korrigiert werden. Auf der anderen Seite darf jedoch die durchaus bestehende Morbidität im Rahmen der Laparotomie und der gelegentlich auftretenden, sehr schweren Komplikationen nicht außer Acht gelassen werden.
