Besonderheiten bei der Diagnostik des kindlichen Thoraxtraumas

Abstract
The respiratory distress syndrom in children after a blunt thoracic trauma indicates an organic lesion inside the thoracic cavity: pneumothorax, haematothorax, lesion of the bronchial tree, posttraumatic atelectasis, emphysema of the mediastinum, cardiac tamponade. Puncture of the thorax is necessary to distinguish between a pneumothorax or a haematothorax. X-ray examinations are important to diagnose a posttraumatic atelectasis, an enlargement of the heart and a rupture of the diaphragm. Bronchoscopy enables to perform a bronchial lavage and to clean the bronchial tree for free ventilation.
Zusammenfassung
Das akute Atemnotsyndrom oder die progredient verschlechternde Atmung bei einem verunfallten Kind deuten auf eine intrathorakale Verletzung hin. In Frage kommen Pneumothorax, Hämatothorax, Bronchusruptur, posttraumatische Lungenatelektasen, Mediastinalemphysem und Herztamponade. Die Punktion des Thorax gibt rasch Aufschluß darüber, ob es sich um einen Hämato- oder Pneumothorax handelt. Die Röntgenuntersuchung orientiert über posttraumatische Atelektasen bzw. Vergrößerung des Herzens oder eine Zwerchfellruptur. Die endobronchiale Endoskopie ermöglicht ein Absaugen von Blut und Sekret aus dem Bronchialraum und ermöglicht die frühzeitige Diagnose von Bronchuseinrissen. Bei der posttraumatischen Atelektase der Lunge hat sich die Bronchiallavage bzw. die endobronchiale Absaugung von Sekret bewährt.
