Summary
We report the case of a 7-year-old male child with a cystic tumour attached to the
caecum close to the ileocaecal valve. The histological examination showed that the
cyst had a ciliated epithelial lining, containing serous and mucous glands, with surrounding
smooth muscle, inner circular and outer longitudinal layers. The cyst was interpreted
as an intestinal duplication lined with respiratory epithelium. This structure and
its location, originating from the midgut, is not in accordance with the typical conception
of the embryogenesis. The clinical picture and treatment are described and compared
to other findings in the literature.
Résumé
Il s'agit d'un rapport concernant un garçon de 7 ans avec une duplication intestinale
abdominale kystique. Le traitement histologique de la tumeur reséquée a fait apparaître
que la paroi du kyste se composait d'une couche musculaire longitudinale et transversale
ainsi que d'un épithélium à cils vibratiles orienté vers la lumière du kyste qui contenait
des glandes muqueuses et séreuses. Il n'est pas compatible avec la représentation
courante de l'évolution embryologique. L'aspect et le traitement de cette duplication
rare de l'appareil digestif y sont présentés et comparés avec la littérature.
Resumen
Se presenta el caso de un muchacho de 7 años que presenta una duplicación quística
del intestino en el abdomen. El estudio histológico del tumor extirpado revela que
la pared del quiste estaba constituida por dos estratos musculares, el primero con
evolución transversal y el segundo dispuesto en sentido longitudinal, así como por
un epitelio ciliado interior con glándulas serosas y mucosas. Este hallazgo no es
compatible con el concepto habitual del desarrollo embriológico. La evolución y el
tratamiento de esta infrecuente duplicación del tracto intestinal se presentan y se
comparan con la bibliografía existente.
Zusammenfassung
Es wird über einen 7-jährigen Jungen mit einem abdominellen zystischen Tumor berichtet.
Die histologische Aufarbeitung zeigte ein dem Lumen zugewandtes cilientragendes Epithel,
dessen Submukosa seröse und muköse Drüsen enthielt. Der Tumor wurde als zystische
Darmduplikatur interpretiert, die mit einem respiratorischen Epithel ausgestattet
ist. Ihr Aufbau und die Lokalisation als Abkömmling des primitiven Mitteldarms sind
mit der gängigen Vorstellung der embryologischen Entwicklung nicht in Einklang zu
bringen. Klinik und Behandlung werden dargestellt und mit der Literatur verglichen.
